The position requires you to facilitate multi-week programs where your clinical judgment drives measurable improvements in patient outcomes. You are the primary provider responsible for maintaining precise, timely, and compliant documentation for every interaction. What you'll do
- Conduct synchronous video visits with patients navigating midlife, perimenopause, and menopause transitions using a dedicated telehealth platform.
- Provide one-on-one virtual nutrition care via phone and video calls to facilitate lifestyle change and enhance patients' wellbeing through evidence-based strategies.
- Work closely with patients in a multi-week program format to support symptom improvement and sustained behavior change in patients experiencing perimenopause.
- Maintain timely and accurate documentation for every patient encounter to ensure clinical clarity and regulatory compliance.
- Follow applicable clinical, documentation, privacy, quality, and compliance standards for visits conducted through Pomelo to protect patient data and safety.
- Utilize commercial health plan enrollment procedures to ensure patient access is maximized within the Pomelo network framework.
- Apply strong clinical judgment to assess complex cases and create individualized nutrition plans without direct supervision during patient visits.
- Demonstrate comfort using telehealth platforms, video conferencing tools, and electronic documentation systems to deliver a seamless patient experience.
- Engage in optional case consultations and provider forums to refine your expertise and expand your clinical knowledge within the network.
- Balance a flexible practice by choosing the days and times you make yourself available for Pomelo patient visits based on personal capacity.
- Uphold the mission of improving outcomes for all mothers and babies by contributing diverse perspectives to a collaborative care environment.
- Accept or decline available visit opportunities with full transparency regarding your licensure, credentialing status, and patient demand.
- Access comprehensive administrative and technical support to navigate Pomelo's systems and resolve any issues that arise during your practice.
- Participate in optional clinical updates to stay current on best practices for metabolic syndrome, bone health, and gut health management.
- Build a sustainable independent practice through Pomelo Care's support for billing, client matching, and administrative functions.
Requirements
- National certification as a registered dietitian (RD) or registered dietitian/nutritionist (RDN) with license to practice in Pennsylvania.
- A minimum of 3 years of experience providing evidence-based nutrition therapy for patients with conditions of metabolic syndrome.
- A minimum of 3 years of experience providing evidence-based nutrition therapy for patients experiencing weight loss and body composition changes.
- A minimum of 3 years of experience providing evidence-based nutrition therapy for patients with osteopenia or osteoporosis.
- A minimum of 3 years of experience providing evidence-based nutrition therapy for patients with gut health concerns, including bloating, IBS, and elimination diets.
- A minimum of 3 years of experience providing evidence-based nutrition therapy for patients following plant-forward or plant-based diets and anti-inflammatory eating patterns.
- Comfortable using telehealth platforms, video conferencing tools, and electronic documentation systems to conduct remote patient care.
- Strong clinical judgment with the ability to work independently during patient visits while maintaining rigorous standards of care.
