About the role
You will design, build, and maintain scalable backend systems that power Hinge Health's AI-driven musculoskeletal care solutions, directly improving member health outcomes. You will own key services within the CareHub platform, collaborating closely with product managers, designers, and cross-functional engineers to deliver new capabilities with high reliability and performance. In this role, you will help advance our mission to transform joint and muscle care through technology by implementing AI-powered workflows that automate clinical processes and enhance care team efficiency. You will work within small, agile, interdisciplinary teams that own their products from development through production, ensuring end-to-end responsibility and rapid iteration. Your contributions will focus on modernizing our platform, improving scalability and maintainability, and enabling clinicians to deliver better care with less friction. You will build and evolve the infrastructure where care happens, ensuring it supports clinicians in delivering personalized, high-quality musculoskeletal care every day.

What you'll do
- Deliver large backend projects that introduce new product capabilities, often acting as a maintainer for critical code repositories and services.
- Break down complex business and clinical requirements into manageable technical tasks, ensuring predictable delivery while identifying and resolving root causes of production incidents.
- Collaborate with product and engineering teams to roadmap and execute ambitious technical strategies, balancing short-term delivery needs with long-term platform scalability and maintainability.
- Mentor and support junior engineers through thoughtful code reviews, real-time pairing, and clear technical guidance to elevate the overall quality of the codebase.
- Design and build AI-powered clinical agents using LangGraph.js, composing multi-node StateGraphs for stateful orchestration of context gathering, LLM reasoning, and action execution within a shared NestJS service.
- Integrate LangSmith for end-to-end AI observability, including prompt versioning and management via LangSmith Hub, automated per-node tracing, evaluation pipelines with golden datasets, and CI-gated quality checks that block regressions before merge.
- Develop and maintain agentic workflows that leverage LLMs such as Claude Sonnet via AWS Bedrock, ensuring secure, reliable, and performant integration with backend services.
- Contribute to the Member-Centric Knowledge Graph and RAG-based retrieval systems, providing clinical context, causal evidence, and relationship awareness that power intelligent decision-making for care teams.
- Proactively improve the maintainability, availability, and performance of team features, conducting architectural reviews and contributing to long-term platform evolution.
- Communicate technical ideas clearly to both technical and non-technical partners, translating complex concepts into actionable plans and documenting solutions for future reference and operational consistency.
Requirements
- Hold a Bachelor's Degree (or equivalent) in Computer Science, Engineering, or a related technical field.
- Bring 5+ years of non-internship full-time professional software development experience in backend systems.
- Demonstrate 3+ years of hands-on work with Node.js and TypeScript, building production-grade services.
- Show experience using AI tools such as GitHub Copilot or Cursor to accelerate development and code quality.
- Have a proven track record of delivering large software projects or features in production environments with high reliability standards.
- Possess 1+ years of hands-on experience building applications with LLMs, including prompt engineering, model integration, and productionizing AI workflows.
- Be comfortable working in a regulated healthcare environment where security and compliance are paramount, with experience engaging with standards such as HIPAA, HITRUST, SOC 2, and CCPA.
- Exhibit strong ownership and operational mindset, embracing DevOps practices and taking pride in supporting your code through deployment and into production.
- Collaborate effectively within cross-functional teams, communicating clearly with product managers, designers, clinicians, and engineers to align on goals and trade-offs.
- Write clean, testable, and maintainable code, with experience in modern software practices including version control, CI/CD, and automated testing.
Nice to have
- Experience with modern frontend frameworks such as React or React Native, enabling full-stack contributions when needed.
- Familiarity with GraphQL, Apollo, and data layer technologies like Postgres, Redis, and RAG systems.
- Prior work in digital health or healthcare technology environments focused on musculoskeletal care or similar clinical domains.
- Experience with containerization and orchestration tools such as Docker and cloud platforms like AWS.
- Background in building or integrating AI agents, including stateful orchestration with LangGraph and observability with LangSmith.
