What you'll do
Coordinate individualized patient care plans and perform care management and care coordination services using Ennoble Care's electronic medical record system.
Initiate frequent contact with patients to provide care coordination, support, and manage compliance with the care management programs to increase positive outcomes.
Document all client communications, whether verbal or written, with accuracy and attention to detail.
Communicate effectively to and from Primary Care Clinician or designee regarding patient emergent needs and life-threatening episodes to ensure comprehensive care plans.
Keep Team Supervisor informed of all issues that pertain to the care plan process and any known or perceived risks to patient well-being.
Collaborate with various cross-organizational areas to satisfy the needs of Ennoble Care's patients, their family members, and partner facilities.
Master the use of technology, including secure email, telephone systems, and electronic medical records, to support daily workflows.
Adhere strictly to documentation protocols and best practices for daily work logs, escalation of client issues, and internal communications.
Exhibit excellent customer service skills that generate positive feedback from both customers and patients.
Support the department as a positive team member by contributing constructively to the success of all colleagues.
Utilize office equipment such as computers, telephones, and copy machines to fulfill job responsibilities efficiently.
Maintain consistent availability to address patient and family concerns during assigned daytime hours in a senior living facility.
