About the role
The hire owns the coordination of patient care activities to ensure high quality service delivery across the organization. They own the management of individual care providers, including scheduling and providing support for caregivers and families. This role owns the responsibility of maintaining open lines of communication between clinicians, staff, and patients to achieve healthcare goals. The hire must demonstrate compassion, experience, and high organization in every interaction. They own the accurate documentation of all client communications and care coordination efforts. This position plays an important part in the ability to provide exceptional care through diligent management of care plans. The hire must adhere to strict documentation protocols and best practices for daily work logs and escalation procedures. They embody the company motto by making decisions that prioritize patient needs and positive outcomes.

What you'll do
Complete individualized patient care plans and perform care management and care coordination services using Ennoble Care's electronic medical record system.
Frequent contact with patients to provide care coordination, support, and manage compliance with the care management programs to increase positive outcomes.
Document all client communications (verbal or written) accurately to maintain clear records and continuity of care.
Communication to and from Primary Care Clinician or designee regarding patient emergent needs and/or life-threatening episodes to ensure comprehensive care plans are complete and accurate.
Keep Team Supervisor informed of all issues pertinent to the care plan process and any known or perceived issues that may impact patient care.
Demonstrate ability to work with various cross-organizational areas to meet the needs of Ennoble Care's patients, their family members, and partner facilities effectively.
Become skilled at using technology including secure email, telephone system, electronic medical records, and other office systems to perform job duties efficiently.
Adherence to documentation protocols and best practices for daily work logs, escalation of client issues, and internal communications to maintain regulatory compliance.
Excellent customer service skills demonstrated by positive feedback from customers and patients through consistent professional interactions.
Contribute as a positive member of the department by supporting all members of the team in a productive and constructive manner to achieve departmental goals.
Utilization of a computer, telephone, copy machine, and other office equipment as necessary to complete daily tasks and responsibilities.
Perform regular care plan reviews and updates in collaboration with clinicians to reflect changes in patient status or treatment goals.
Coordinate transportation and scheduling needs for patients to ensure timely access to care services within the network regions.
Assist in training new team members on documentation procedures and technology systems to uphold consistent standards across the care team.
Monitor patient satisfaction metrics and provide feedback to leadership for continuous improvement of service delivery.
Requirements
Must be comfortable with speaking on the phone for large amounts of the day to coordinate care and communicate with multiple stakeholders.
Must be compassionate and empathetic towards our patients, always demonstrating exceptional customer service in every interaction.
Ability to take accurate notes to document each task in a timely manner to ensure proper billing and continuity of care.
Ability to multitask between different patients and workstreams while remaining organized and efficient with time to meet demanding schedules.
Ability to thrive in a fast-paced environment where priorities may change rapidly and adaptability is essential.
Must be able to work full-time, Monday through Friday, 8:30am-5:00pm, on-site in Marlton, New Jersey without exception.
Must be proficient in using a computer, including Outlook and other Microsoft Office programs to manage documentation and communications.
Knowledge of basic healthcare terms, conditions, roles, and basic care principles to perform duties effectively and safely.
Candidate must be able to pass a drug screen, background check, have a positive attitude, adapt positively to change, be a team player, and be willing to learn new skills on a continuous basis.
Must have the right to work and eligibility to work in the United States without sponsorship for the position as outlined in the job description.
