This position is based on-site at our Illinois location and involves providing outpatient mental health services through our telehealth program. What you'll do

• Provide outpatient mental health services to patients through our telehealth platform, ensuring accessible care for diverse populations
• Conduct thorough assessments to evaluate patients' mental health and substance abuse issues
• Develop individualized treatment plans tailored to each patient's needs and goals
• Deliver evidence-based therapeutic interventions in accordance with best practices and ethical standards
• Collaborate with clients to establish realistic goals and monitor their progress over time
• Maintain accurate, timely, and HIPAA-compliant documentation of all clinical interactions and treatment plans
• Coordinate care with other healthcare providers, community organizations, and support services to ensure comprehensive patient care
• Support patients in managing mental health challenges, substance use issues, and related concerns
• Participate actively in team meetings, case consultations, and ongoing training to improve clinical skills
• Uphold confidentiality and privacy standards in line with HIPAA regulations
• Engage in continuous quality improvement initiatives to enhance service delivery
• Contribute to organizational efforts to expand access to mental health services, especially for Medicaid and Medicare populations
• Assist in community outreach efforts to promote mental health awareness and reduce stigma
• Stay current with developments in mental health treatment, telehealth technology, and relevant policies
• Support organizational goals of diversity, equity, and inclusion in service delivery
• Provide culturally competent care, respecting patients' backgrounds and preferences
• Utilize telehealth technology effectively, including video communication and internal communication tools
• Participate in supervision and professional development activities as required

Requirements

• Master's degree in social work or a related mental health field
• Valid licensure as a Licensed Social Worker (LSW) in Illinois
• Unrestricted authorization to work in the United States; employer sponsorship not supported or required
• Ability to work from a private, HIPAA-compliant home workspace, ensuring patient confidentiality
• Fluency in English; proficiency in Spanish preferred, with additional languages a plus
• Experience providing outpatient mental health services, preferably in community or Medicaid settings
• Strong interpersonal and communication skills, with a compassionate approach
• Comfort with telehealth platforms, video communication, and digital documentation tools
• Ability to conduct comprehensive assessments and develop effective treatment plans
• Knowledge of mental health and substance abuse diagnoses and treatment modalities
• Ability to work independently and as part of a multidisciplinary team
