About the role

You will own the end-to-end clinical experience for assigned patients, ensuring that medical, behavioral, and social needs are met through timely and high-quality virtual interactions. You will design and execute personalized care plans that align with the value-based goals of Nest while actively coordinating with cross-functional teams to remove barriers to access. You will serve as a key clinical voice in translating patient and population insights into actionable strategies for product and service development. You will mentor peers and new hires on best practices for in-home and virtual care delivery within a fast-moving environment. You will leverage data and patient feedback to continuously refine workflows and elevate the member experience. You will act as a trusted partner to families, guiding them through complex care navigation with empathy and clarity. You will uphold the highest standards of clinical documentation and communication to support sustainable, scalable care delivery. You will collaborate with leadership to define success metrics and drive outcomes that reinforce the mission of making care radically accessible.

What you'll do

Conduct comprehensive virtual assessments for adult and pediatric patients to develop accurate diagnoses and treatment pathways.

Coordinate multi-disciplinary care plans that integrate primary care, mental health, nutrition, and social services for holistic member support.

Lead same-and-next-day acute visit scheduling and clinical decision-making to ensure timely interventions.

Perform chronic condition management by monitoring patient progress and adjusting care strategies in partnership with the care team.

Execute vaccination protocols and laboratory workflows to maintain up-to-date clinical records and compliance.

Navigate complex transitions of care to reduce hospital readmissions and promote continuity across settings.

Utilize telehealth platforms and digital tools to deliver high-quality in-home and remote clinical services.

Engage in proactive outreach to identify at-risk families and connect them with appropriate resources and social navigation services.

Apply clinical judgment to prioritize cases based on urgency, risk, and population health needs.

Document all interactions with precision to support billing, quality metrics, and continuous improvement initiatives.

Communicate effectively with payers and partners to align care delivery with contractual and regulatory requirements.

Support the implementation of new care pathways and protocols as Nest scales across new markets and service lines.

Promote health equity by addressing social determinants of health and connecting families to community-based resources.

Serve as an ambassador for the Nest care model, gathering insights that inform product development and member experience enhancements.

Requirements

You must hold a current, active license to practice medicine or nursing in the state where you are physically located.

You must have demonstrable experience delivering primary care to diverse populations in clinical settings.

You must possess strong clinical judgment and the ability to make timely decisions in a virtual care context.

You must be proficient in using electronic health records and telehealth platforms to manage patient information securely.

You must have the capacity to work flexible hours, including evenings and weekends, to accommodate patient needs across time zones.

You must be able to independently manage complex cases while collaborating effectively with interdisciplinary teams.

You must demonstrate excellent written and verbal communication skills for coordinating care and engaging with stakeholders.

You must be committed to ongoing learning and adaptation in a rapidly evolving healthcare environment.

Nice to have

Prior experience with value-based care models and population health management.

Experience working with pediatric and adult patients in a combined clinical capacity.

Knowledge of social determinants of health and community resource linkage.

Travel is not required, but the position may involve occasional in-person training or meetings.
