About the role

Nest Health is seeking a full-time Advanced Practice Provider to deliver comprehensive medical, social, and behavioral care to high-risk families in the Flagstaff area. You will function as a primary care lead within an interdisciplinary team, balancing both virtual consultations and in-person visits at community facilities or patient homes. In this capacity, you will own the clinical management of a defined patient panel while ensuring that care plans align with the social determinants of health unique to the region. You are expected to exercise diagnostic autonomy within a collaborative framework, integrating medical, behavioral, and social data to guide treatment. The role requires a proactive approach to population health, focusing on preventive services and the coordination of complex care needs. What you'll do

• Manage a panel of families by providing longitudinal primary care, preventive services, and medication management.
• Conduct wellness exams, acute care visits, and chronic condition monitoring for both pediatric and adult patients.
• Collaborate with MDs, RNs, LCSWs, and psychiatrists during daily huddles and weekly case conferences.
• Document clinical care plans and assist families in navigating social resources like housing, food, and transportation.
• Perform diagnostic testing, venipuncture, and medication reconciliation as required.
• Participate in community outreach and provide rotating virtual on-call coverage during evenings and weekends.
• Utilize data-driven approaches to identify gaps in care and implement targeted interventions for high-risk individuals.
• Coordinate referrals to specialty services and ensure timely follow-up on recommended actions.
• Maintain strict adherence to clinical protocols while adapting care strategies to meet the specific cultural and logistical needs of the community.
• Support quality improvement initiatives by tracking outcomes and contributing to best practice development within the care delivery team.

Requirements

• Graduate of an accredited Physician Assistant or Nurse Practitioner program.
• Valid, unrestricted Arizona medical license.
• Current BLS/CPR certification.
• Ability to travel between service locations in Flagstaff and Show Low using a reliable personal vehicle.
• Must possess a valid driver's license, auto insurance, and the ability to lift 25 pounds.
• Capacity to work in a fast-paced startup environment with high autonomy.
• Demonstrated commitment to serving vulnerable populations through a background in community or rural health settings.
• Compliance with all HIPAA regulations and Nest Health standards for privacy and data security.
• Willingness to undergo background checks and professional reference verification as part of the hiring process.
• Availability to work a schedule that includes evenings and weekends to support virtual on-call responsibilities.

Nice to have

• At least 2 years of clinical experience serving adult and pediatric populations with chronic or behavioral health conditions.
• Previous experience in a leadership role within an interdisciplinary care team.
• Prior work experience in a startup setting.
