What you'll do
Assess and implement individualized behavioral support plans through direct 1:1 client interaction in home and community settings based on Mental Health and TBS assessments.
Develop and execute positive behavior programs tailored to the specific needs of qualified clients, focusing on skill-building and de-escalation techniques.
Champion rigorous compliance with established safety protocols and confidentiality policies to protect client dignity and privacy at all times.
Serve as the primary program monitor for assigned cases, meticulously completing all required forms, accountability documentation, and incident reports.
Execute behavioral interventions precisely as prescribed by clinical guidelines and treatment plans to ensure consistency and effectiveness.
Analyze and document client progress using objective quality control measures, identifying trends and adjusting strategies as needed.
Maintain comprehensive and organized case records for every assigned client, coordinating participation in case reviews and goal planning sessions.
Arrange necessary transportation, prepare detailed required documentation, and ensure accurate reporting to support seamless service delivery.
Cultivate and sustain professional relationships with consumers, parents, care providers, co-workers, and community partners to represent Hope Services positively.
Act as a mandated reporter, adhering to legal and ethical obligations regarding the safety and welfare of vulnerable individuals.
Provide intervention and consultation to clients and families, serving as an advocate to navigate challenges and access resources.
Perform a wide range of additional duties as assigned to ensure the efficiency, integrity, and continuity of program services.
Support scheduling logistics and documentation workflows to optimize team performance and client satisfaction.
Engage in ongoing professional development to stay current with best practices in therapeutic behavioral support and mental health.
Requirements
Earn a Bachelor's degree in Psychology or its equivalent as approved by the San County Mental Health (SCCMH) board.
Demonstrate a proven interest and ability to understand and relate effectively to children with developmental or mental health disabilities and behavioral challenges.
Possess working knowledge of handling aggressive or assaultive behaviors in a safe and controlled manner.
Show a consistent ability to relate in a positive and constructive manner with clients, their families, the community, and diverse staff members.
Exhibit strong skills in observing, evaluating, documenting, and communicating client performance and social behavior both verbally and in writing.
Apply knowledge of psychological and mental health disabilities, alternative communication systems, and assistive technology to inform support strategies.
Utilize critical thinking skills in complex decision-making situations, supported by excellent organizational and record-keeping abilities.
Maintain current certification in CPR and First Aid as a non-negotiable condition of employment.
Demonstrate a deep knowledge of and commitment to the principles of normalization and enhancing community integration for adults with developmental disabilities.
Approach scheduling and task prioritization with flexibility, positivity, and cooperation within a consumer-driven environment.
Meet all eligibility criteria and hard bars as defined by the position requirements without exception.
