Key facts
What you'll do
- Assess member needs comprehensively across physical health, mental health, substance use disorder, oral health, palliative care, memory care, trauma-informed care, social supports, housing, and referral and linkage to community-based services and supports.
- Oversees the development, implementation, and evaluation of client care plans and goal settings using SMART frameworks to ensure measurable progress.
- Offer services flexibly where the member resides, seeks care, or finds most easily accessible, including office-based, telehealth, or field-based services to meet individual needs.
- Connect clients to essential social services, community resources, and other supports that address social determinants of health and holistic well-being.
- Advocate on behalf of the client with healthcare professionals, including primary care physicians, specialists, pharmacists, and social services providers to ensure care alignment.
- Utilize evidence-based practices consistently, such as Motivational Interviewing, Harm Reduction, and Trauma-Informed Care principles to guide interventions and support.
- Conduct proactive outreach and engagement activities to facilitate linkage to the ECM program and meticulously log all activities in the Client Relationship Management (CRM) system for accurate tracking.
- Evaluate client progress on an ongoing basis and update care plans and SMART goals to reflect changing needs and circumstances.
- Provide mental health promotion and psychoeducation to members and their families to build resilience and support networks.
- Arrange necessary transportation services, such as ACCESS programs, to ensure reliable and timely access to care.
- Complete all required documentation, including outcome measures, within the strict timeframes established by individual care plans and regulatory requirements.
- Maintain up-to-date, accurate, and confidential patient health records in the Electronic Medical Record (EMR) system and any other relevant business systems.
- Complete monthly reporting with precision to ensure full program compliance, regulatory standards, and internal quality assurance.
- Attend all mandatory training sessions and professional development opportunities as assigned to maintain competency and certification.
Requirements
- Willing and able to work a consistent schedule Monday through Friday from 8:30 am to 5:00 pm Pacific Time, with the understanding that occasional evenings and weekends may be required.
- Possess an active, unrestricted Licensed Clinical Social Worker (LCSW), Licensed Marriage and Family Therapist (LMFT), or Licensed Professional Clinical Counselor (LPCC) license specific to the state of California.
- Bring a minimum of 1 year of professional experience in a care manager, care navigator, or community health worker role, specifically focused on supporting vulnerable populations.
- Demonstrate a solid working knowledge of government and community-based resources related to social determinants of health, including housing, food security, and transportation.
- Exhibit excellent oral and written communication skills to effectively interact with members, families, and multidisciplinary professional teams.
- Possess positive interpersonal skills that foster trust, respect, and collaboration with diverse individuals and communities.
- Must have general computer proficiency and a working knowledge of Google Workspace, Microsoft Office Suite, and standard internet navigation for daily tasks.
- Bilingual capability in English and Spanish is highly preferred to ensure effective communication and rapport with a broader member population.
