About the role
Found is transforming personalized weight care with an evidence-based platform that combines modern medicine, behavior change support, personalized coaching, and a supportive community. As an early part of Found's newest offering, you will own the expansion of RD services and personalized nutritional coaching for members with insurance primarily through synchronous consultations. You will conduct health assessments, manage lab screenings, and handle diagnosis management related to obesity while documenting and providing ongoing follow-up care. In this role, you will partner closely with prescribing clinicians to adapt care plans based on medication changes, weight-loss progress, and insurance coverage. You will use motivational interviewing, SMART goal-setting, and CBT-informed coaching to build patient self-efficacy and long-term lifestyle change. The position requires technological savvy, a commitment to lifelong learning, and independent practice experience in obesity medicine and telemedicine. You will practice with high standards of personal and professional integrity, ensuring confidentiality while coordinating care across Found's internal systems to develop long-term patient relationships.

What you'll do
- Conduct synchronous nutrition counseling sessions with patients (60min initial and 30min followup) to support their individual medication assisted weight loss journeys.
- Proactively manage common GLP-1 and AOMs side effects (nausea, constipation, reflux, reduced appetite, risk of lean-mass loss) through personalized nutrition strategies that preserve muscle mass, support adequate protein and fiber intake, and enable sustainable weight loss.
- Apply evidence-based behavior change techniques (motivational interviewing, SMART goal-setting, CBT-informed coaching) to build self-efficacy and long-term lifestyle change.
- Adapt care plans in response to changes in medication or dosing, insurance coverage, weight-loss plateaus, or evolving patient goals, in partnership with the prescribing clinician.
- Followup with patients via asynchronous messaging to address questions and concerns in a timely and professional manner.
- Coordinate care with Found clinicians and the member support team leveraging Found's internal EMR provider messaging function to ensure continuity of care.
- Serve patients with high standards of personal and professional integrity and ethics, maintaining a high level of confidentiality at all times.
- Complete session documentation, including ADIME notes, patient communication, and care team follow-up messages, in a timely manner for processing and reporting.
- Leverage Found's technology platform to keep patients engaged in their care journey between sessions through reminders, educational content, and progress tracking.
- Stay up to date on current nutrition topics, research, and best practices to incorporate into patient care and education.
- Support the growth of RD services by participating in quality improvement initiatives and sharing insights from patient interactions.
- Communicate effectively with cross-functional teams to align clinical strategies with product and operational goals.
- Monitor patient outcomes and adjust nutritional recommendations based on lab results, progress notes, and treatment responses.
- Act as a clinical expert and patient advocate, ensuring that nutrition care aligns with evidence-based guidelines and patient preferences.
Requirements
- RD/RDN credentialed with the CDR (active and in good standing).
- Licensed in multiple states, including some combination of: NY, IN, CA, PA, NC, GA, TX, SC, MD, OH, CT, MA, ME, NH, NJ, VT, VA, WA, DC.
- Registered Dietitian certification and current license(s) required prior to start date.
- Previous experience in obesity medicine and telemedicine (synchronous and asynchronous) strongly preferred.
- Demonstrated ability to work independently while maintaining strict adherence to clinical protocols and documentation standards.
- Strong understanding of pharmacotherapy for weight management and nutrition-related side effects.
- Excellent verbal and written communication skills to engage patients and collaborate with clinical teams.
- Proficient in using technology platforms for telehealth, electronic medical records, and secure messaging.
- Commitment to upholding patient confidentiality and compliance with HIPAA and other relevant regulations.
- Willingness to participate in ongoing training and quality improvement activities to enhance program delivery.
