What you'll do
- Initiate contact with patients through various channels such as phone calls, SMS, video conferencing, or face-to-face meetings to connect them with necessary healthcare resources.
- Establish and nurture relationships with patients to enhance their engagement and trust in the healthcare process.
- Evaluate patient data sourced from Electronic Health Records to prioritize care needs and monitor preventive health measures effectively.
- Maintain clear and professional communication with primary care physicians regarding updates on patient status and care plans.
- Ensure comprehensive documentation of all patient interactions to maintain accurate records.
- Actively participate in weekly meetings with the care team to discuss and coordinate support strategies for patients.
- Collaborate with internal teams to refine operational workflows and enhance overall efficiency.
- Assist in the development of outreach strategies to engage underserved populations within the community.
- Provide feedback on patient experiences to help improve service delivery and patient satisfaction.
- Stay informed about community resources and services that can benefit patients.
- Conduct follow-up communications with patients to ensure they are receiving the necessary support and care.
- Engage in continuous learning and professional development to stay updated on best practices in patient outreach and care coordination.
Requirements
- Prior experience in customer service or sales roles.
- A professional background in healthcare delivery, particularly in communicating with medical professionals.
- Familiarity and proficiency with Electronic Health Records systems.
- Excellent verbal and written communication skills, with the ability to adapt messages for different audiences, including both healthcare providers and patients.
- Strong organizational skills with the ability to work independently and manage time effectively.
- Proficiency in Google Workspace applications.
- Must be a resident of the Greater Hampton Roads area.
- Access to a personal vehicle for travel within the Greater Richmond or Hampton Roads regions.
- Willingness to participate in a mandatory three-week paid virtual training program during the onboarding process.
Nice to have
- Experience in patient outreach, care coordination, or operations within outpatient clinics.
- Multilingual abilities to better serve diverse patient populations.
- Knowledge of managed care systems or health insurance processes.
- Understanding of the Medicaid demographic and familiarity with the Richmond or Hampton Roads communities.
- Previous experience working with underserved populations or in community health initiatives.
