What you'll do

• Initiate contact with patients through various channels such as phone calls, text messages, video conferencing, or face-to-face visits to connect them with essential health services.
• Establish and nurture relationships with patients to enhance their engagement and build trust in the care process.
• Analyze patient data sourced from Electronic Health Records (EHR) to identify and prioritize their healthcare needs.
• Maintain regular communication with primary care physicians to provide updates on patient status and care plans.
• Ensure meticulous documentation of all patient interactions to maintain accurate records and facilitate continuity of care.
• Participate in weekly team meetings to discuss care strategies and coordinate efforts among team members.
• Collaborate with the internal team to identify areas for improvement in operational workflows and enhance overall efficiency.
• Support patients in navigating the healthcare system, providing guidance and resources to help them access necessary services.
• Monitor patient follow-ups to ensure compliance with care plans and address any barriers to health.
• Engage in community outreach initiatives to raise awareness about available health resources and services.
• Assist in the development of educational materials that inform patients about their healthcare options and rights.

Requirements

• Exceptional verbal and written communication abilities, with the capacity to tailor your approach to suit both healthcare providers and patients.
• Strong organizational skills and the capability to work autonomously while managing multiple tasks.
• Previous experience in sales or customer service roles that demonstrate your ability to engage with diverse populations.
• Professional experience with Electronic Health Records, ensuring familiarity with patient data management.
• Background in a healthcare delivery setting, particularly in roles that involve interaction with medical professionals.
• Candidates must reside in the Greater Cleveland area and possess a vehicle for local travel.
• Proficient in using Google Workspace applications for documentation and communication purposes.
