Key facts
What you'll do
- Initiate contact with patients through various channels such as phone calls, text messages, video conferencing, or face-to-face visits to connect them with essential health services.
- Establish and nurture relationships with patients to enhance their engagement and build trust in the care process.
- Analyze patient data sourced from Electronic Health Records (EHR) to identify and prioritize their healthcare needs.
- Maintain regular communication with primary care physicians to provide updates on patient status and care plans.
- Ensure meticulous documentation of all patient interactions to maintain accurate records and facilitate continuity of care.
- Participate in weekly team meetings to discuss care strategies and coordinate efforts among team members.
- Collaborate with the internal team to identify areas for improvement in operational workflows and enhance overall efficiency.
- Support patients in navigating the healthcare system, providing guidance and resources to help them access necessary services.
- Monitor patient follow-ups to ensure compliance with care plans and address any barriers to health.
- Engage in community outreach initiatives to raise awareness about available health resources and services.
- Assist in the development of educational materials that inform patients about their healthcare options and rights.
Requirements
- Exceptional verbal and written communication abilities, with the capacity to tailor your approach to suit both healthcare providers and patients.
- Strong organizational skills and the capability to work autonomously while managing multiple tasks.
- Previous experience in sales or customer service roles that demonstrate your ability to engage with diverse populations.
- Professional experience with Electronic Health Records, ensuring familiarity with patient data management.
- Background in a healthcare delivery setting, particularly in roles that involve interaction with medical professionals.
- Candidates must reside in the Greater Cleveland area and possess a vehicle for local travel.
- Proficient in using Google Workspace applications for documentation and communication purposes.
