As the San Bernardino/Orange County area Community Health Worker (CHW) Lead for Waymark, you will lead our community health workers in the region. Reporting to the Market Operations Lead, you will supervise and manage a team of CHWs - helping them implement their day-to-day priorities, providing coaching and support around patient care, and managing performance to team goals. You will also develop and steward community partnerships and resource information to support patient care, and impact how we design, structure, and implement the CHW experience for our employees, spanning recruitment, hiring and training, deployment, mentorship, resourcing, providing pathways to advancement, and advocating for CHWs across our organization and externally.

What you'll do
Provide direct supervision of the day-to-day activities of CHWs in Orange and San Bernardino counties, including helping them establish and implement their schedules and priorities.
Lead weekly team huddles with a multidisciplinary team - including CHWs, licensed clinical social workers, pharmacists, and care coordinators - to coordinate patient care.
Coach, mentor, and support CHWs to ensure satisfaction and sustainability within the role, and to ensure competency validation, including by managing one-on-one and group meetings with the CHW team and occasionally shadowing fieldwork.
Manage staffing schedules to ensure patient coverage and continuity of care within the local CHW team.
Review data dashboards to manage CHW performance relative to goals and identify strengths and opportunities.
Collaborate with the Operations and Learning & Development teams to hire, train, deploy, and provide ongoing support to local CHWs.
Navigate technology systems to supervise CHW documentation and ensure quality care.
Represent Waymark in the community and build and nurture community partnerships.
Champion patient-centered care practices by coordinating closely with clinical and administrative stakeholders to align services with community needs.
Facilitate resource mapping and information sharing to connect patients with local services and support networks.
Support the implementation of new workflows and tools to improve CHW efficiency and effectiveness in the field.
Gather feedback from CHWs and community partners to inform continuous improvements in programs and operations.
Monitor local health trends and system changes to adapt CHW strategies and ensure responsive care delivery.
Promote a culture of learning and improvement by documenting best practices and sharing insights across the CHW team.
Ensure compliance with organizational standards, regulatory requirements, and ethical guidelines in all community interactions.
Requirements
Demonstrate experience working as a Community Health Worker or in a similar frontline service role supporting vulnerable populations.
Show a proven track record of managing and leading teams, with experience supervising staff or volunteers in a healthcare or community setting.
Possess strong knowledge of community resources, social determinants of health, and healthcare navigation within San Bernardino and Orange County contexts.
Have the ability to interpret data dashboards and performance metrics to inform decisions and drive improvements.
Exhibit excellent communication, interpersonal, and cultural competency skills to engage with diverse communities and stakeholders.
Display strong problem-solving and critical-thinking abilities to navigate complex patient needs and system barriers.
Commit to upholding patient confidentiality, ethical standards, and regulatory compliance in all activities.
Be comfortable using technology platforms and tools for documentation, communication, and scheduling in healthcare environments.
Nice to have
Experience partnering with community-based organizations and stakeholders to coordinate resources and services.
Background in healthcare operations, public health, or social services, with familiarity with Medicaid systems and reimbursement models.
Experience with quality improvement methods or data-driven decision-making in community health settings.
Training or experience in coaching, mentoring, or adult education methodologies.
Practical notes
This is a full-time role with regular team huddles and coordination across multidisciplinary partners. Travel within the San Bernardino and Orange County regions is required to supervise field activities and meet partners. Candidates must be eligible to work in the United States without sponsorship at this time.
