About the role
Vynca is seeking a compassionate and skilled Social Worker to become a vital member of our Clinical Care Delivery Team. This position focuses on delivering crucial psychosocial support, counseling, and care coordination for patients facing serious illnesses. You will collaborate with an interdisciplinary team, utilizing a hybrid approach that includes both in-person visits and telehealth services to ensure comprehensive patient care.

What you'll do
- Conduct thorough psychosocial evaluations to determine the needs and strengths of patients and their families.
- Provide grief and bereavement support, including anticipatory grief counseling and assistance with funeral arrangements.
- Identify and address mental health issues such as depression, anxiety, and stress experienced by caregivers.
- Assess safety concerns, including suicidal thoughts, abuse, neglect, and unsafe living environments.
- Actively participate in weekly interdisciplinary team meetings to review and refine patient care strategies.
- Manage a caseload of medical social work cases for a designated group of patients and their families.
- Address a range of psychosocial, practical, financial, and legal issues affecting patients and their support networks.
- Assist patients and families in accessing and utilizing available community resources effectively.
- Guide patients and families through complex social and healthcare systems.
- Mediate disputes between families and healthcare providers to ensure cohesive care.
- Conduct financial needs assessments and provide guidance on navigating Medicare, Medicaid, Disability, Social Security, and Veterans Affairs benefits.
- Evaluate spiritual and medical needs, ensuring timely communication with the interdisciplinary team.
- Establish and nurture relationships with community resources to facilitate patient support.
- Develop comprehensive social work care plans, identifying issues, setting goals, and outlining interventions and visit frequency.
- Facilitate advance care planning, including the completion of advance directives and POLST forms.
- Ensure all documentation is completed within 48 hours of service.
- Collaborate with the interdisciplinary team and external organizations, including facilitating hospice referrals.
- Participate in community outreach and educational initiatives to promote Vynca services.
- Perform additional duties as assigned by the Clinical Manager or Chief Medical Officer.
- Travel throughout the assigned New Jersey territory for in-person patient visits, training sessions, or company events.
Requirements
- Must possess an active, unrestricted Social Work license (LCSW) in New Jersey, with a willingness to obtain licensure in other states as needed.
- A minimum of 2 years of experience in healthcare social work is required.
- Candidates should hold either a Master of Social Work (MSW) degree or a Bachelor of Social Work (BSW) degree with relevant experience.
- Experience in hospice, palliative care, oncology, or home health settings is preferred.
- A solid understanding of cultural competency relevant to the patient population is essential.
- Excellent oral and written communication skills, along with strong interpersonal abilities, are necessary.
- Proficient in general computer skills, including familiarity with MS Office, G-Suite, and internet applications.
- Experience with Electronic Medical Record (EMR) systems and business platforms like Salesforce is required.
- Availability to work Monday through Friday from 8:30 AM to 5:00 PM Eastern Time, with flexibility for occasional evenings and weekends.
Nice to have
- Certification in palliative care is a plus.
- Bilingual capabilities, particularly in English and Spanish, are advantageous.
- Possession of certifications such as CHPN, C-ASWCM, or similar, or the ability to obtain them is preferred.
- Previous experience with remote work is beneficial.

Practical notes
This position operates in a hybrid model, requiring travel across northern New Jersey for in-person patient visits, in conjunction with telehealth services throughout New Jersey and potentially other states. Candidates must reside within a 25-mile radius of the assigned territory. The selection process may involve a phone screening, online assessments, interviews, an offer, and background/reference checks. A background check, which may include drug testing or other health screenings, is mandatory prior to employment. Employees in patient-facing roles must be vaccinated against influenza, with accommodations available for religious or medical reasons. Verification of identity and work eligibility through E-Verify will be required upon hiring.
