What you'll do
- Assess member needs across physical health, mental health, substance use disorder, oral health, palliative care, memory care, trauma-informed care, social supports, housing, and referral and linkage to community-based services and supports.
- Oversee the development of client care plans and the establishment of measurable goal settings in collaboration with the individual and their support network.
- Offer services where the member resides, seeks care, or finds most easily accessible, including office-based, telehealth, or field-based services.
- Connect clients to other essential social services and community supports that address their holistic needs.
- Advocate on behalf of the client with health care professionals, including primary care providers and specialists, to ensure coordinated and effective treatment.
- Utilize evidence-based practices, such as Motivational Interviewing, Harm Reduction, and Trauma-Informed Care principles to guide interventions and build trust.
- Conduct outreach and engagement activities to facilitate linkage to the ECM program and meticulously log all activities in the Client Relationship Management (CRM) system.
- Evaluate client progress on an ongoing basis and update SMART goals to reflect changes in needs and achievements.
- Provide mental health promotion and psychoeducation to empower members and their families.
- Arrange transportation services, such as ACCESS, to ensure reliable and timely access to care.
- Complete all required documentation, including outcome measures, within the strict timeframes established by individual care plans and regulatory standards.
- Maintain up-to-date patient health records in the Electronic Medical Record (EMR) system and other relevant business systems to ensure accuracy and compliance.
- Complete monthly reporting to ensure full program compliance and support continuous quality improvement.
- Attend all assigned training sessions to maintain certification and stay current with best practices.
Requirements
- Possess an active, unrestricted ACSW, LCSW, LMFT, or LPCC license in California, which is mandatory for practice.
- Hold at least 1+ year of professional experience as a care manager, care navigator, or community health worker supporting vulnerable populations, with 2 or more years being preferred.
- Demonstrate a willingness and ability to work a schedule of Monday-Friday from 8:30am to 5:00pm, with flexibility for both in-the-field and remote work.
- Show willingness and ability to work additional hours, including potential evenings and weekends, to meet the demands of the role and client needs.
- Maintain a current and working knowledge of government and community resources related to social determinants of health to effectively connect clients with support.
- Exhibit excellent oral and written communication skills to interact effectively with clients, families, and professional partners.
- Demonstrate positive interpersonal skills to build rapport and trust with diverse individuals and communities.
- Possess a valid driver's license and have reliable transportation to travel throughout the assigned territory.
- Have general computer proficiency and a working knowledge of Google Workspace, MS Office, and standard internet navigation.
- Be bilingual in English and Spanish, a qualification that is highly preferred for effective client engagement.
Practical notes
This is a hybrid position that requires traveling throughout the Los Angeles County area up to 5 days per week. Candidates wishing to be considered must reside within 20-miles of the assigned territory due to frequency of travel.
