Our Clinical Care Specialists provide direct care and education to patients with chronic cardiometabolic conditions in collaboration with our Physicians. What you'll do
- Provide comprehensive diabetes, hypertension, hyperlipidemia, and weight loss care, education, and support for members through chat, phone, and video calls following internal protocols.
- Create thorough consults aligned with internal protocols for 9amHealth physicians and the provider team.
- Collaborate with Patient Care Advocates to ensure member success.
- Develop tailored nutrition and lifestyle plans for members based on their medical conditions, goals, and preferences, ensuring they align with evidence-based guidelines for managing diabetes, hypertension, hyperlipidemia, and weight loss.
- Collaborate with dedicated pharmacists while utilizing your experience and comfort in managing medications related to diabetes, hypertension, hyperlipidemia, and weight loss to provide comprehensive care and guidance to members.
- Track member progress through regular check-ins (as needed), assessing the effectiveness of interventions, and adjusting plans as needed based on clinical outcomes and member feedback.
- Provide motivational interviewing, coaching, and counseling to support sustainable behavior changes in members regarding diet, exercise, and stress management.
- Utilize member data and health metrics to inform care recommendations, helping members interpret lab results and track outcomes like weight, body composition, blood pressure, and cholesterol levels.
- Maintain accurate and thorough member documentation, ensuring that all care actions, recommendations, and follow-ups are recorded in accordance with internal and legal standards.
- All care team members are required to work 2 weekends per quarter.
Requirements
- Minimum Qualifications
- Licensed Registered Dietitian Nutritionist plus Certified Diabetes Care and Education Specialist (CDCES) or Board Certified-Advanced Diabetes Management (BC-ADM) or eligible for certification.
- Strong knowledge of and experience in providing care for chronic cardiometabolic conditions, specifically diabetes, hypertension, hyperlipidemia, and obesity.
- Minimum 5 years of experience in direct patient care, preferably in a clinical setting or via telehealth, with a focus on chronic disease management.
- Experience in medication adjustment and using an EMR.
- Demonstrated ability to build strong, trusting relationships with patients through compassionate, tailored communication.
- Experience working as part of an interdisciplinary team, with demonstrated ability to collaborate closely with physicians, patient care advocates, and other healthcare professionals.
- Experience providing care through virtual platforms (chat, phone, video), with comfort using digital health tools and patient management systems.
- A quiet, dedicated workspace free from distractions to ensure patient privacy and maintain confidentiality.
- Reliable high-speed internet connection to support phone, video, and messaging communications without interruption.
- Flexible and resilient; you understand that in a growing company, today's process may look different tomorrow and you're okay with that.
