About the role
You are a person who enjoys helping others and you are currently seeking fulfillment in your professional life. In this role, you own the mission of providing individualized support to consumers so they can identify personal barriers and create meaningful community participation. You develop methods for accessing activities and services in community settings that are defined by the consumer's Person-Centered Plan and Individualized Program Plan. You work closely with family members to identify needs and translate those needs into actionable plans that promote independence. You facilitate positive interactive experiences within community settings to help consumers feel welcomed and capable. You act as a dedicated advocate on behalf of consumers served, ensuring their voices are heard and their rights are respected. You implement behavioral intervention plans when needed to assure consumer safety and promote learning within everyday contexts. You represent HOPE in the community in a professional manner through every interaction and commitment.

What you'll do
Provide individual support in the home or community setting as identified in the consumer's Individualized Personal Plan.
Work closely with family members to identify needs and develop plans that foster long-term growth and stability.
Develop opportunities for consumers to participate as valued and contributing members of the community through tailored activities.
Facilitate and develop activities that build relationships, skills, and experience in the community while traveling with and transporting clients.
Develop and expand consumer participation in typical and generic community activities chosen by the consumer, including transporting the consumer to various community locations.
Act as an advocate on behalf of consumers served by ensuring their preferences and needs guide all support strategies.
Complete required forms and accountability documentation, including incident reports, case recordings, IPPs, and other mandated records.
Maintain a professional relationship with consumers, family members, coworkers, and others in every environment.
Represent HOPE in the community in a professional manner through consistent, ethical, and compassionate interactions.
Implement behavioral intervention plans when needed to assure consumer safety and promote learning during daily routines.
Coordinate consumer programming with Center's activities, services, and staff to ensure seamless integration and support.
Adhere to safety agency practices and policies at all times to maintain a secure and reliable support environment.
Act as a mandated abuse reporter by recognizing and reporting any signs of abuse or neglect promptly and appropriately.
Perform other duties as assigned to assure efficiency of program services and meet organizational goals.
Requirements
4 year degree preferred, plus one year of related experience working with people with disabilities, or an approved combination of experience and education.
Possession of a valid California driver's license and clear driving record is mandatory for this position.
CPR/First-aid Certification is required, and Hope can provide the training if certification is currently not held.
A belief that all people are valued members of the community is essential for success in this role.
An understanding of the principles of normalization and a commitment to community integration must guide all professional interactions.
Ability to work independently and cooperatively as a member of a diverse team is necessary for consistent performance.
Excellent communication and documentation skills are required, including the use of computerized word programs and databases.
Knowledge and understanding of alternative communication systems and assistive technology must be applied when supporting consumers.
CPR and First Aid certification must be current, with the ability to respond to first aid injuries and perform Heimlich and CPR when alone with consumers in the community.
A positive and cooperative attitude toward a consumer driven service system is required when working with developmentally disabled adults.
Flexibility with scheduling and the ability to work in a consumer driven environment is an essential part of the position.
Strong interpersonal skills are required to effectively work with a diverse population across varying backgrounds and needs.
