About the role
You will champion the principle of normalization in every interaction you facilitate. This role owns the direct implementation of community based services that honor the consumer's Person Centered Plan. You are responsible for developing opportunities that transform routine activities into valued community participation. You will act as a consistent advocate ensuring that each individual's voice guides the support they receive. In this position you will build genuine relationships while modeling positive community interaction for consumers. You own the completion of accurate and timely accountability documentation for every engagement. You will implement behavioral strategies when necessary to uphold safety and promote skill acquisition. You will coordinate programming with center staff to ensure seamless service delivery.

What you'll do
Coordinate community based activities that align with each consumer's Individualized Personal Plan.
Develop and facilitate opportunities for consumers to engage as valued members of their local community.
Transport and travel with consumers to ensure participation in personally meaningful community activities.
Expand consumer involvement in typical and generic community options driven by personal choice.
Act as a dedicated advocate to represent consumer interests and uphold their rights.
Complete required forms, case recordings, incident reports, IPPs and all mandated accountability documentation.
Maintain professional relationships with consumers, families and community partners at all times.
Implement behavioral intervention plans to support safety, learning and positive outcomes.
Coordinate programming and logistics with Hope Services center staff and community resources.
Act as a mandated reporter for abuse, neglect and self harm in every situation.
Perform additional duties as assigned to promote the efficiency and integrity of program services.
Support the delivery of services in a flexible manner that adapts to consumer needs and schedules.
Ensure accurate communication with assistive technology and alternative communication systems when required.
Promote a consumer driven environment through active listening and responsive support strategies.
Requirements
High school diploma, GED, or equivalent, plus two years of related experience or an approved combination of experience and education.
Possession of a valid California driver's license and clear driving record.
A belief that all people are valued members of the community.
An understanding of the principles of normalization and a commitment to community integration.
Ability to work independently and cooperatively as a member of a team.
Excellent communication and documentation skills including use of computerized word program and data base.
Knowledge and understanding of alternative communication systems and assistive technology.
CPR and First Aid certification which Hope Services can provide necessary training for if needed.
Positive and cooperative attitude toward a consumer driven service system working with developmentally disabled adults.
Ability to be flexible with scheduling and work in a consumer driven environment.
Strong interpersonal skills to work with a diverse population.
Commitment to maintaining confidentiality and adhering to agency policies and procedures.
Ability to pass a background check as required by agency policy and regulatory standards.
Nice to have
CPR and First Aid certification prior to start date.
Experience in developmental disabilities or mental health support settings.
Familiarity with local community resources and service options.
Skills in de-escalation techniques and positive behavior support.
Ability to speak multiple languages relevant to the community served.
Experience with documentation software and electronic health records.
Practical notes
This is a full-time position with extensive local travel required.
The role operates in community settings and may require work in various local and home communities.
Candidates must be able to lift and carry supplies for consumer supports as needed.
Position may involve climbing stairs occasionally to reach consumers.
The work environment includes exposure to varied climatic conditions such as heat, cold, humidity and sun.
Sitting for extended periods may occur during meetings and office based tasks.
Candidates must be capable of working independently and in close collaboration with coworkers.
Driving is required; public transportation may be used when necessary.
This position represents a community based role primarily in the Mountain View area.
