About the role
This role centers on providing person centered planning and support to adults with developmental disabilities within the community. The hire will own the full lifecycle of individual service planning and implementation while ensuring compliance with Division of Developmental Services policies. They will act as a primary liaison between individuals, families, and a network of community providers to coordinate seamless service delivery. This position requires strong advocacy skills to secure necessary supports and remove barriers to community participation. The role emphasizes building trusting relationships that promote independence, choice, and inclusion for each individual served. Success in this position will be measured by improved quality of life outcomes and strengthened community connections for the people we support.

What you'll do
Coordinate and facilitate person centered planning processes to identify goals, preferences, and needed supports for each individual.
Conduct comprehensive assessments and synthesize findings into Individual Service Plans and Plans of Care that align with DDS policies and timelines.
Ensure active participation of individuals in all stages of planning, decision making, and service review to uphold self determination.
Maintain accurate, timely, and compliant documentation of services, progress, and outcomes according to organizational policy and regulatory requirements.
Build and sustain positive professional relationships with individuals, families, caregivers, and community providers through clear communication and follow through.
Arrange and coordinate community based services such as Section 8 housing, SSI applications, and other necessary supports while advocating for timely access.
Schedule and coordinate assessments, service conferences, and appointments across multiple community locations as required by the service plan.
Provide linkage to additional providers and resources, identifying needs and proactively scheduling related appointments and interventions.
Monitor service implementation and progress toward goals, adjusting plans in collaboration with the team and individuals when necessary.
Support community integration by facilitating participation in activities that enhance social connections, skill development, and personal well‑being.
Promote reliability and consistency by maintaining a set schedule, being on time for all commitments, and communicating proactively about any changes.
Serve as a trusted point of contact for problem solving, ensuring that issues are addressed promptly and collaboratively with all stakeholders.
Contribute to continuous improvement by gathering feedback, tracking outcomes, and sharing insights to strengthen the overall support system.
Requirements
Bachelor's degree or equivalent experience in human services, social work, psychology, or a related field.
Demonstrated experience working with adults with intellectual disabilities or acquired brain injury in community based settings.
Proven ability to manage a caseload using an outreach model while balancing multiple priorities and deadlines effectively.
Current valid driver's license and access to a reliable vehicle suitable for community based travel.
Ability to work a community based schedule that includes Tuesday through Saturday, 9am to 5pm, with flexibility to support individuals across their required hours.
Capacity to travel within the service areas of Lawrence, Lowell, Burlington, and Arlington to meet individuals where they live and receive support.
Commitment to working from the main office location in Lexington, MA, while supporting community activities as needed.
Understanding of and adherence to Eliot policies, DDS regulations, and MRC documentation and billing standards in all service activities.
Nice to have
Experience coordinating services with Section 8 housing authorities and state disability agencies.
Familiarity with evidence based practices in community integration and adult developmental services.
Skills in using data driven approaches to track individual outcomes and service effectiveness over time.
Ability to provide training or mentorship to peers or new team members on person centered planning and community engagement.
Practical notes
Position requires Tuesday through Saturday work, 9am to 5pm, with potential for adjusted hours based on individual needs and service requirements.
Travel is essential across the Lawrence, Lowell, Burlington, and Arlington service areas and from the main office in Lexington, MA.
Candidates must be eligible for employment in the United States and able to meet the demands of a community based case management role.
