About the role
This role owns the direct facilitation of community integration for consumers as outlined in their personalized plans. You own the development of opportunities that allow consumers to engage as valued members within their local community. The position requires you to facilitate and model interactive relationships between consumers and community members through shared activities. You own the responsibility of implementing behavioral strategies to ensure safety and learning when necessary. This role acts as a dedicated advocate for every consumer served by the organization. You own the completion of all required documentation and accountability forms to maintain compliance. The position requires you to coordinate programming with existing center staff and activities. You represent Hope Services professionally in every community interaction.

What you'll do
Coordinate group and individual support sessions in the community based on individualized personal plans.
Develop and expand participation in typical community activities chosen directly by the consumer.
Facilitate interactive relationships between consumers and general community members.
Transport and travel with consumers throughout the local community during support sessions.
Build skills and relationships through structured community-based activities.
Act as a mandated reporter for abuse while maintaining professional boundaries.
Implement behavioral intervention plans to promote consumer safety and learning.
Complete necessary forms, including IPPs and case recordings, with accurate documentation.
Coordinate scheduling and programming with center staff and other stakeholders.
Perform additional duties as assigned to ensure the efficiency of program services.
Maintain professional relationships with consumers, families, and community partners.
Adapt to flexible scheduling needs within a consumer-driven service environment.
Represent Hope Services in a professional manner during all community engagements.
Support the goals of the organization through various assigned responsibilities.
Requirements
High school diploma, GED, or equivalent is required along with two years of related experience.
Possession of a valid California driver's license is mandatory with a clear driving record.
A belief that all people are valued members of the community is a hard requirement.
An understanding of the principles of normalization and a commitment to community integration is required.
The ability to work independently and cooperatively as part of a team is essential.
Excellent communication and documentation skills must be demonstrated consistently.
Knowledge and understanding of alternative communication systems and assistive technology are required.
CPR and First Aid certification is required, though Hope Services can provide necessary training.
A positive and cooperative attitude toward a consumer driven service system is mandatory.
Flexibility with scheduling and the ability to work in a consumer driven environment is required.
Strong interpersonal skills are necessary to work effectively with a diverse population.
The ability to travel extensively around town and within the consumer community is required.
Comfort using public transportation where possible is a necessary part of the role.
Exposure to varied environmental conditions including heat, cold, humidity, and dampness is expected.
Practical notes
This is an On-Call/Substitute engagement.
The position requires extensive travel including local community transport.
Work is primarily community-based in the Gilroy/Hollister area.
The role may involve lifting, carrying supplies, and walking or standing for extended periods.
Some tasks will require visual and auditory acuity while supporting consumers in the community.
Climbing stairs occasionally may be necessary to reach consumers in home settings.
The work environment includes both solitary tasks and close collaboration with colleagues.
Sitting for periods of time in meetings and use of office equipment will occur.
Produce work on computers and standard office equipment as part of daily responsibilities.
