Community Health Outreach Coordinator

About the role

You will own the execution of community activation plans in partnership with Santa Clara Family Health Plan to drive enrollment in Foodsmart's foodcare program. This role requires you to build authentic relationships with community-based organizations and local leaders to establish trust and credibility on the ground. You will translate complex nutrition benefits into clear, accessible information for diverse populations during face-to-face interactions. You will leverage data and feedback to refine outreach tactics and ensure that every activation moves measurable enrollment outcomes. You will act as the voice of Foodsmart in community spaces, representing the brand with professionalism and empathy at every touchpoint. You will identify and prioritize high-impact events and locations to maximize member reach within limited part-time hours. You will document activities and insights to support continuous improvement of the outreach strategy. You will collaborate closely with internal teams to align field activities with program goals and reporting requirements.

What you'll do

Coordinate community events and on-site activations in alignment with Santa Clara Family Health Plan objectives to increase Foodsmart program enrollment.

Develop and maintain partnerships with local organizations, faith-based groups, and neighborhood leaders to create a trusted referral network.

Design and implement outreach strategies that engage hard-to-reach populations across Silicon Valley, including Latinx, Vietnamese, Filipino, and South Asian communities.

Conduct culturally and linguistically appropriate education sessions about Foodsmart benefits and how to use them.

Navigate local community landscapes to identify optimal locations for enrollment drives and health education activities.

Collect and report real-time data on outreach activities and enrollment progress to inform decision-making.

Provide in-person support to help members complete enrollment and SNAP-related paperwork with accuracy and care.

Serve as a resource for partner organizations by answering questions and offering guidance on Foodsmart program features.

Utilize empathy and active listening to understand member barriers and tailor conversations to individual needs.

Maintain a visible and professional presence in the community that reflects Foodsmart's mission and values.

Requirements

Must be at least 18 years of age at the time of hire.

Must have the legal right to work in the United States and not require work authorization sponsorship from the company for this position now or in the future.

Must reside in or be willing to relocate to Santa Clara County, California, to fulfill the responsibilities of this role.

Must have reliable transportation and a valid driver's license, and be willing to use personal vehicle for authorized business travel.

Must be able to travel up to 25 working hours per week across various locations in Santa Clara County and the surrounding Bay Area.

Must pass a background check and comply with Foodsmart's policies on confidentiality and data security.

Must demonstrate strong written and verbal communication skills in English, with bilingual ability in Spanish, Vietnamese, Filipino, or other relevant languages considered a significant asset.

Must have a deep understanding of or demonstrated ability to learn the cultural nuances and community dynamics in Silicon Valley diverse communities.

Must be comfortable using digital tools and platforms to capture data, manage schedules, and communicate with team members.

Nice to have

Preferred candidates will have prior experience in community health, outreach, or enrollment roles within a healthcare or social service environment.

Experience working with SNAP, EBT, and public assistance programs will be viewed favorably.

Knowledge of the food as medicine philosophy and interest in nutrition-driven healthcare is encouraged.

Experience collaborating with dietitians, healthcare providers, or public health professionals is a plus.

Practical notes

This is a part-time contractual position with up to 25 working hours each week.

The selected candidate must be willing and able to travel to various locations across Santa Clara County and surrounding Bay Area as needed for work activities.
