What you'll do
- Serve as a primary point of contact for clients, providing personalized support and guidance throughout their healthcare journey.
- Conduct comprehensive assessments to understand clients' needs, preferences, and healthcare goals.
- Educate clients about available healthcare options, including insurance plans, services, and community resources.
- Assist clients in completing necessary applications for health insurance and other healthcare services.
- Collaborate with healthcare providers and community organizations to ensure clients receive timely and appropriate care.
- Monitor client progress and follow up regularly to address any concerns or changes in their healthcare needs.
- Develop and maintain strong relationships with clients, fostering trust and open communication.
- Stay informed about changes in healthcare policies, insurance regulations, and community resources to provide accurate information.
- Participate in training and professional development opportunities to enhance your skills and knowledge in healthcare navigation.
- Document client interactions and maintain accurate records in compliance with organizational policies and regulations.
- Contribute to team meetings and discussions, sharing insights and strategies for improving client services.
- Assist in the development of outreach programs aimed at increasing community awareness of available healthcare resources.
Requirements
- Bachelor's degree in a relevant field such as healthcare administration, social work, or public health.
- A minimum of 3 years of experience in healthcare navigation, case management, or a related role.
- Strong understanding of healthcare systems, insurance processes, and community resources.
- Excellent interpersonal and communication skills, with the ability to build rapport with clients from diverse backgrounds.
- Proficiency in using electronic health records (EHR) and other relevant software applications.
- Ability to work independently and as part of a collaborative team.
- Strong organizational skills and attention to detail, with the ability to manage multiple tasks effectively.
- Commitment to maintaining client confidentiality and adhering to ethical standards in healthcare.
Nice to have
- Certification in healthcare navigation or case management.
- Experience working with underserved populations or in community health settings.
- Bilingual skills, particularly in Spanish or other languages commonly spoken in the Denver area.
- Familiarity with telehealth services and digital health tools.
