About the role
This role owns the on-site coordination of care for long-term care residents assigned to Ennoble Care within one or more partnered facilities in the Manassas, Virginia area. The hire will use the Ennoble Care EMR to manage and document resident care activities while maintaining regular, proactive engagement with residents to support care plans and transitions. They will serve as the primary liaison between residents, their families, facility staff, and Ennoble Care providers to ensure timely communication of needs and condition changes. The hire will cultivate trusted, consistent relationships that reflect the family-practice model and embody the motto "To Care is an Honor." They will follow established workflows and escalation protocols to accurately document all interactions and contribute positively to team collaboration. This position supports the broader mission of delivering remote patient monitoring and proactive care coordination in a hands-on, in-person capacity. The role is embedded within the facility to ensure continuity, relationship-building, and patient-centered support.

What you'll do
Coordinate care for long-term care residents using Ennoble Care's EMR.
Maintain regular, proactive, on-site engagement with residents to support care plans, follow-ups, and care transitions.
Collaborate closely with Ennoble Care providers, within the long-term care setting.
Serve as a key liaison between providers, facility staff, residents, and families.
Communicate patient needs, changes in condition, and urgent concerns to the clinical team in a timely manner.
Accurately document patient interactions and care coordination activities.
Follow established workflows, documentation standards, and escalation protocols.
Contribute positively to team collaboration and strong facility partnerships.
Provide on-site presence that mirrors a family-practice model of consistent, familiar care.
Support the implementation and monitoring of remote patient monitoring (RPM) initiatives for assigned residents.
Assist in the coordination of care transitions to ensure continuity across care settings.
Act as a point of contact for families and facility staff regarding care plan updates and questions.
Uphold Ennoble Care standards by demonstrating professionalism in all in-person interactions.
Promote patient-centered care by anticipating needs and aligning with facility schedules and workflows.
Requirements
Legally authorized to work in the United States of America.
Active, unrestricted clinical certification or licensure, such as: Patient Care Technician (PCT), Certified Nursing Assistant (CNA), Medical Assistant (MA) / Certified Medical Assistant (CMA), Licensed Practical Nurse (LPN), Registered Nurse (RN), or comparable credential.
Minimum 1 year of patient-facing experience in a long-term care, skilled nursing, or similar clinical setting.
Comfortable communicating proactively and professionally throughout the day via Microsoft Teams, email, phone, and in person.
Cultivate and maintain positive, approachable relationships with residents, families, providers, and facility staff, while demonstrating a consistent, dependable presence within the long-term care setting.
Well-developed organizational and time-management skills, paired with an optimistic approach and initiative-driven mindset.
Strong working knowledge of healthcare terminology and care coordination principles.
Proficiency with Microsoft 365, including Outlook, Word, and Excel.
Ability to work full-time, Monday through Friday, 8:00 a.m. to 4:30 p.m. EST, (or thereabouts, contingent on facility hours) in an on-site role, based in the Reston, VA area, with a primary focus on the Warrenton vicinity.
No weekends.
No on-call.
Daytime hours.
Seven (7) company-paid holidays annually.
Transportation criteria: Valid, unencumbered driver's license, consistent, reliable transportation to fulfill the duties of this position, current auto insurance with the ability to provide proof, and note that travel between facilities may be required.
Ability to pass a background check.
Practical notes
Hours: Full-time, Monday through Friday, 8:00 a.m. to 4:30 p.m. EST, with no weekends or on-call expectations.
Travel: Travel between facilities may be required.
