Intensive Case Manager

About the role

At Eliot Community Human Services, the Intensive Case Manager role exists to deliver clinically focused, evidence-based interventions that promote resilience, empowerment, and recovery for adults with severe and persistent mental illness. You will own the end-to-end management of complex cases within the Adult Community Clinical Services (ACCS) framework, ensuring that every intervention remains person-centered and aligned with individual recovery goals. Your work will center on providing side-by-side support that builds problem-solving capacity, teaches new skills, and models optimism in real-world settings. This role requires collaboration with a multidisciplinary team to coordinate seamless care that respects each person's dignity and autonomy while fostering natural community connections and empowering clients toward integration in employment, leisure, and daily living. The position demands emotional steadiness and reliability, including the capacity to provide on-call coverage during evenings and weekends as part of a rotating schedule. Through this work, you will directly contribute to lasting change by helping individuals move toward stability, self-determination, and long-term recovery.

What you'll do

Coordinate comprehensive, person-centered treatment plans that reflect the unique strengths and goals of each adult client.

Deliver evidence-based, side-by-side interventions that emphasize problem-solving, skill-building, and real-time modeling of adaptive behaviors.

Champion community outreach efforts in collaboration with the multidisciplinary ACCS team to engage adults with severe and persistent mental illness.

Support the development and execution of recovery plans that prioritize integration into community life, including natural supports and daily routines.

Empower individuals to assume control over their treatment, leisure activities, employment pathways, and ongoing personal recovery journey.

Maintain current knowledge of clinical best practices and translate them into practical, optimistic encouragement during direct client interactions.

Provide structured guidance that helps clients build capacity, resilience, and independence within both residential and community-based environments.

Accept on-call responsibilities as part of a rotating schedule to ensure consistent coverage during evenings and weekends.

Facilitate regular communication and coordination with team members to align strategies and share critical updates about client progress.

Demonstrate cultural humility and responsiveness by honoring diverse backgrounds, perspectives, and lived experiences in every interaction.

Requirements

Hold a Bachelor's or Master's degree in Psychology, Social Work, or a closely related field as the foundation for practice.

Bring prior experience working directly with adults who have serious and persistent mental illness in clinical or community settings.

Possess a valid driver's license issued in the state of Massachusetts to support community-based engagement and travel.

Location, schedule, and compensation

Location: Lynn

Engagement: Full-time

Schedule: Monday through Friday, 8:00 AM to 4:00 PM, with rotating weekend and evening shifts as required.

Compensation: $52,000 to $56,000 USD annually.

Bilingual candidates fluent in a needed language may be eligible for a 6% salary increase.
