You will own the complex coordination of care for youth and families in crisis within a multidisciplinary FIT Team. This role requires you to act as the central hub linking clinicians, families, providers, and community systems. What you'll do

Coordinate comprehensive care planning with the FIT team to align services with family needs and safety goals.

Perform in-depth face-to-face intake and assessment by gathering detailed information from youth, family, and all involved providers.

Develop, review, and revise assessments, values-based treatment plans, and comprehensive safety and crisis management plans collaboratively.

Initiate care planning activities that translate assessment findings into actionable, measurable steps for stability.

Provide proactive phone outreach to new referrals to assess needs and connect them with appropriate resources.

Deliver targeted case management and care coordination through in-person meetings, virtual sessions, and telephone contacts.

Support coordination of care across behavioral health, medical, and community providers including schools, DCF, DMH, DDS, DPH, DESE/LEA, DYS, Probation Office, Courts, and medical providers.

Offer emergency response and intensive outreach to stabilize children and adolescents within their home during psychiatric crises.

Maintain accurate, timely, and confidential documentation of all contacts, interventions, and progress in accordance with program and regulatory standards.

Serve as the primary point of contact for the youth and family, ensuring clear communication and consistent follow-through.

Collaborate with family partners and clinicians to monitor progress and adjust plans as needed to meet changing needs.

Promote access to wellness and support resources, including fitness benefits and childcare referrals, to sustain family well-being.

Uphold ethical standards and values-based practice in every interaction with families and partners.

Participate in rotation of weekend coverage and after-hours on-call responsibilities to ensure continuous support.

Requirements

Hold a Bachelor's degree in human services or a related field as the educational foundation for practice.

Possess two years' experience working with the target population of youth, children, and families in mental health or related settings.

Demonstrate experience navigating child and family service systems and advocating for family members within behavioral health systems.

Maintain a valid driver's license and provide reliable transportation for work and community-based service delivery.

Be prepared to work hours between 8am and 8pm, including evenings, weekends, and after-hours on-call as part of the program schedule.

Commit to working 40 hours per week within the structured schedule that supports continuity of care.

Hours are fixed at 40 per week between 8am and 8pm.

Candidates must be able to commute to Lynn reliably and hold valid vehicle registration and insurance for community travel.
