What you'll do

Conduct comprehensive assessments of clients to determine their needs and develop personalized treatment plans tailored to their specific circumstances.
Provide ongoing case management services, ensuring clients have access to necessary resources such as housing, healthcare, and vocational training.
Facilitate individual and group counseling sessions to support clients in their recovery process and promote healthy coping mechanisms.
Collaborate with healthcare providers, social services, and community organizations to coordinate care and support for clients.
Monitor client progress and adjust treatment plans as needed, ensuring that clients are on track to meet their recovery goals.
Engage in outreach activities to connect with potential clients and raise awareness about available services for individuals with substance use disorders.
Maintain accurate and up-to-date records of client interactions, treatment plans, and progress notes in compliance with organizational policies and regulations.
Participate in regular team meetings and training sessions to enhance skills and stay informed about best practices in substance use treatment.
Advocate for clients' needs within the healthcare system and assist them in navigating challenges related to their recovery.
Develop and implement community education programs aimed at reducing stigma around substance use disorders and promoting recovery resources.
Build and maintain relationships with community stakeholders to foster collaboration and enhance service delivery.
Evaluate the effectiveness of outreach initiatives and adjust strategies based on feedback and outcomes.

Requirements

Bachelor's degree in social work, psychology, or a related field; a master's degree is preferred.
A minimum of 2 years of experience in case management or direct service provision in a mental health or substance use treatment setting.
Knowledge of substance use disorders, treatment modalities, and recovery principles.
Strong communication skills, both verbal and written, with the ability to engage effectively with diverse populations.
Proven ability to work independently and as part of a multidisciplinary team.
Familiarity with community resources and services available to individuals with substance use disorders.
Proficiency in using electronic health record systems and other relevant software.
Valid driver's license and reliable transportation for outreach activities.
Certification in substance use counseling or related credentials is a plus.

Nice to have

Experience working with marginalized populations or in a community-based setting.
Bilingual skills, particularly in Spanish, to better serve the diverse community of Los Angeles.
Previous involvement in program development or community outreach initiatives.
Knowledge of harm reduction strategies and practices.
