Requirements

- Excellent communication and interpersonal skills, with the ability to build rapport with diverse patient populations.

- Proficiency in electronic health record (EHR) systems and case management software.
- Strong organizational skills and attention to detail, with the ability to manage multiple cases simultaneously.
- Knowledge of relevant regulations and compliance standards in healthcare.
- Certification in case management (CCM or ACM) is a plus.
