Requirements
- Bachelor's degree in social work, nursing, public health, or a related field is required.
- A minimum of 2 years of experience in case management, patient advocacy, or a similar role within a healthcare setting.
- Strong understanding of healthcare systems, insurance processes, and community resources available to patients.
- Excellent communication skills, both verbal and written, with the ability to convey complex information clearly and compassionately.
- Proven ability to work collaboratively in a team-oriented environment while also demonstrating strong independent decision-making skills.
- Proficiency in using electronic health record (EHR) systems and case management software to document and track patient information.
- Strong organizational skills and attention to detail, with the ability to manage multiple cases and priorities simultaneously.
- Knowledge of relevant regulations and standards governing patient care and case management practices.
- Ability to work effectively with diverse populations and demonstrate cultural competence in interactions with patients and families.
