About the Role

This position involves guiding the full client care journey, from initial intake through final documentation, while consistently upholding the highest standards of clinical excellence. You will operate autonomously within a structured, collaborative environment, shaping personalized treatment plans and communicating clearly with your care team. The role is responsible for driving high-quality behavioral health outcomes within partnered community programs.

Key Responsibilities

You will set daily schedules and prioritize cases based on capacity and acuity levels to ensure efficient and effective service delivery. You will conduct thorough biopsychosocial evaluations to guide accurate diagnosis and the development of appropriate treatment pathways. You will design individualized care plans that reflect best practices and are aligned with measurable treatment goals. You will coordinate with medical colleagues to integrate behavioral health strategies into broader integrated care models. You will utilize telehealth technology and secure documentation systems to conduct consistent and reliable virtual sessions. You will monitor client progress over time, adapting interventions based on measurable responses and structured feedback. You will support continuity of care through detailed handoffs, clear and comprehensive notes, and reliable follow-up practices. You will partner with community organizations to remove access barriers and connect clients to necessary resources. You will maintain all licensing, credentialing, and compliance documentation, benefiting from full administrative support.
