You will own the clinical leadership and operational performance of a dedicated pod of advanced practice providers within your assigned market, directly impacting population health outcomes and family experiences. This role requires you to balance hands-on patient care with strategic people management, ensuring that Nest's value-based care model is executed with precision and compassion. You will be accountable for driving clinical quality, adherence to guidelines, and the professional growth of your team, all while maintaining a collaborative and data-informed approach. You will work closely with regional and market leadership to align on goals, identify opportunities, and solve complex care delivery challenges. In this position, you will also contribute your clinical expertise through virtual patient visits, demonstrating Nest's integrated model of care. You will act as a coach, mentor, and quality steward, translating Nest's mission into daily practice for every family served.

What you'll do

• Serve as the clinical owner of your pod, supervising and coordinating the activities of advanced practice providers to ensure high-quality, timely, and family-centered care.
• Partner with the Market or Regional Medical Director to validate clinical decisions, refine protocols, and support ongoing education for APPs across the market.
• Collaborate with the market VP/General Manager and Health Services Director as part of the leadership triad to align workflows, prioritize quality improvement projects, and advance operational excellence.
• Analyze performance data, chart reviews, and patient feedback to identify trends, drive corrective actions, and sustain high standards of care.
• Develop and implement training programs for new and existing APPs, ensuring consistent application of Nest tools, workflows, and clinical standards.
• Conduct regular supervision through ride-alongs, chart audits, and feedback sessions to support skill development and adherence to best practices.
• Lead quality improvement initiatives focused on chronic disease management, care transitions, vaccination coverage, and adherence to value-based care metrics.
• Participate actively in interdisciplinary case conferences and team huddles to coordinate care plans, manage risk, and address social determinants of health.
• Provide a limited but meaningful panel of direct virtual and in-home patient visits for pediatric and adult populations to maintain clinical acuity and continuity.
• Act as a clinical resource during periods of provider absence or surge demand, ensuring continuity of care and minimizing service disruption.
• Partner with social services and navigation teams to connect families with community resources, removing barriers to care and improving social outcomes.
• Champion patient and family engagement strategies that promote trust, transparency, and shared decision-making across all encounters.
• Support the implementation and optimization of clinical documentation, coding, and compliance processes to protect Nest's value-based contracts.
• Contribute to the design and execution of pilot programs that test new care delivery models, technology integrations, and service expansions.
• Communicate performance results, success stories, and operational insights to leadership, payers, and stakeholders in a clear and actionable manner.

Requirements

You must hold an active, unencumbered license as a Nurse Practitioner or Physician Assistant in the state of Louisiana. You must have current certification in Advanced Cardiac Life Support (ACLS) and Pediatric Advanced Life Support (PALS) or be eligible for certification upon hire. You must have at least three years of progressive clinical experience in primary care, pediatrics, or a related specialty with a demonstrated track record of high-quality patient care. You must have a minimum of two years in a leadership or supervisory role, overseeing clinical staff, residents, or trainees in a healthcare setting. You must demonstrate proficiency with electronic health records, telehealth platforms, and clinical data analytics tools commonly used in value-based care environments. You must have a deep understanding of medical necessity, utilization management, and documentation standards required by managed care and government payers. You must be able to pass a criminal background check, verify professional licensure, and comply with all health system and regulatory compliance standards. You must be physically capable of performing in-home visits, including lifting and carrying lightweight medical equipment, navigating various residential environments, and traveling between appointments. You must be available to work a flexible schedule that includes evenings, weekends, and on-call hours to meet patient and operational needs across the New Orleans market.

Nice to have

Prior experience with population health management, care coordination, and social determinants of health integration is preferred. Experience working with payer contracts, risk stratification, and quality reporting programs is highly valued. Familiarity with Nest's technology stack, including any EHR, patient engagement, or data visualization tools, is a plus. Background in training adult learners and delivering clinical coaching in a startup environment is encouraged. Experience collaborating with community-based organizations and cross-functional teams to address health equity and access barriers is appreciated.

Practical notes

This role is full-time and requires on-site presence in the New Orleans, Louisiana market, with the expectation of occasional travel between neighborhoods and partner sites as needed. Candidates must be eligible to work in the United States without sponsorship for this position. The schedule is variable and may include early mornings, evenings, weekends, and on-call responsibilities to ensure continuity of care and response to emergent needs. In-home clinical duties may require physical stamina and adaptability to diverse living environments. Compliance with all regional, state, and federal regulations governing telehealth, licensure, and controlled substance prescribing applies to this role.
