The role offers a compensation rate of $23.06 per hour. It operates across all of Santa Clara County. This is a full-time engagement designed for individuals who find purpose in helping others and seek professional fulfillment.

Position Summary

The Independent Living Skills Trainer provides individualized services to HOPE clients. The core responsibility involves assessing and remediating barriers that inhibit successful community integration. This may include delivering training and intervention within the client's home and community environments, particularly in community employment settings. The trainer works closely with clients and their families to define training objectives and ensure ongoing success.

Essential Functions

The position requires the consistent execution of the following essential duties.

1. Delivering one-on-one assessments, training, and follow-up services in home and community settings as detailed in the client's Individual Service Plan (ISP).

3. Ensuring strict compliance with established safety and confidentiality standards.

4. Acting as a program monitor for assigned clients, which involves completing all required forms and accountability documentation, including incident reports, case recordings, termination records, client attendance, billing, floor files, and other mandated paperwork.

5. Performing task analysis and time studies to support wage evaluations in accordance with Department of Labor regulations.

6. Assessing and documenting client progress while performing quality control checks.

7. Maintaining comprehensive records for all assigned client cases. This includes participating in case reviews and goal planning sessions, as well as scheduling transportation and preparing necessary documentation and reports.

8. Building and maintaining professional relationships with consumers, parents, care providers, co-workers, and other relevant parties. The trainer serves as a representative of HOPE Services within the community.

9. Acting as an advocate for clients by providing necessary intervention and consultation.

10. Fulfilling the role of a mandated abuse reporter.

11. Performing additional duties as assigned to ensure the efficient operation of program services.

Required Qualifications

Candidates must possess a High School diploma, G.E.D., or equivalent credential. also, a minimum of one year of related experience working with individuals with disabilities in a rehabilitation environment is required.

Required Knowledge and Skills

1. Demonstrating a genuine interest and the ability to understand and relate to adults who have developmental or multiple disabilities.

2. Establishing positive and constructive relationships with clients, their families, community members, and staff.

3. Accepting supervision and working cooperatively as a team member with all Supported Living, Mobility, and other agency personnel.

4. Observing, evaluating, documenting, and communicating client performance and social behavior effectively in both verbal and written formats.

5. Applying knowledge of physical and mental disabilities, alternative communication systems, and assistive technology.

6. Utilizing critical thinking skills in decision-making scenarios and maintaining strong organizational and record-keeping abilities.

7. Holding a current certification in CPR and First Aid.

8. Understanding and committing to the principles of normalization to enhance the community integration of adults with developmental disabilities.

9. Approaching scheduling and task prioritization with flexibility, positivity, and cooperation within a consumer-driven environment.

Environmental Conditions

The position involves an extensive amount of travel within the local community. It requires prolonged periods of standing and walking. The trainer will be exposed to dust, noise, pollens, and various weather conditions. The role includes writing tasks, computer work, and the use of a variety of alternative transportation modes.
