You will operate as a key clinical partner within the Foodsmart Nutrition Network, owning the full virtual delivery of Medical Nutrition Therapy for members across diverse care settings. This role requires you to start with the member and work backwards, ensuring that every interaction aligns with our customer-first value of placing the individual at the center of the care journey. You will act with urgency and hold high standards, using data and evidence-based practices to guide meaningful behavior change and improve health outcomes. What you'll do
Conduct comprehensive virtual Medical Nutrition Therapy sessions with members, focusing on personalized clinical nutrition care and behavior change.
Utilize dietary assessments to identify individual needs and create tailored nutrition plans that support healthier food choices and sustainable outcomes.
Guide members through online food ordering platforms, ensuring they can effectively use digital tools to complement their nutrition goals.
Partner with national and regional retailers to facilitate access to nutritious food options, with a focus on those that accept SNAP and EBT benefits.
Support members in understanding and maximizing food benefits, including assistance with SNAP enrollment and ongoing program management.
Leverage data and clinical insights to hold high standards of care, using performance metrics to refine the member journey and improve health results.
Collaborate with internal teams to integrate nutrition counseling with meal planning solutions that optimize ingredients for home and on-the-go consumption.
Deliver education and coaching that empowers individuals and families to make affordable, healthy choices regardless of economic status.
Apply behavior change frameworks to foster lasting improvements in dietary patterns and overall well-being for members in employer-sponsored plans and public programs.
Act as a trusted clinical resource, translating complex nutrition science into practical, actionable steps that members can implement in daily life.
Champion health equity by serving populations who are disproportionately affected by diet-related diseases, particularly low-income workers and underserved communities.
Engage proactively with Foodsmart's digital platform to ensure seamless delivery of care across virtual touchpoints and integrated tools.
Contribute to the growth and impact of the Foodsmart Nutrition Network by maintaining strong communication and collaboration with partner stakeholders.
Demonstrate ownership of the member journey from initial contact through ongoing support, ensuring alignment with Foodsmart's mission and values.
Requirements
You must hold active certification as a Registered Dietitian in the state of New York.
You must be currently licensed to practice as a Dietitian in New York.
You must reside within the United States and be able to perform contracted work within the US jurisdiction.
You must have the legal right to work in the United States without sponsorship at this time.
You must possess a deep understanding of Medical Nutrition Therapy and its application in virtual care settings.
You must demonstrate strong knowledge of nutrition science, dietary guidelines, and behavioral strategies for long-term change.
You must be able to interpret dietary assessments and translate them into individualized nutrition care plans.
You must have experience engaging with diverse populations, including those in Medicaid managed care and employer-sponsored health plans.
Nice to have
Experience partnering with national and regional retailers, particularly those accepting SNAP or EBT payment methods.
Understanding of food benefits administration and SNAP enrollment processes.
Background in delivering nutrition care through digital health platforms or virtual counseling environments.
Familiarity with employer-sponsored health plans, Medicare Advantage, and Medicaid managed care structures.
Experience supporting members facing food insecurity or diet-related chronic conditions.
