About the role

Ennoble Care is seeking an experienced, Per Diem, Hospice Social Worker for our Kansas City, KS area. In this role, you will go directly to the home of the patient to provide essential emotional and practical support during end-of-life care. You will own the psychosocial assessment process and ensure it is completed within the required timeframe for every new admission. This position requires you to actively participate in interdisciplinary goal setting and care planning sessions to align services with patient and family priorities. You will observe and document the evolving emotional and social needs of patients as their condition progresses. You will also act as a vital link between the patient, family, and the broader medical team to ensure a coordinated approach. Furthermore, you will help families navigate community and personal resources to strengthen their support systems. Finally, you will continuously develop your professional expertise to uphold the highest standards of clinical practice and compassion.

What you'll do

Conduct a comprehensive psychosocial assessment within 5 days of admission to identify emotional, social, financial, and environmental factors.

Participate in interdisciplinary goal setting meetings to develop a cohesive and patient-centered plan of care.

Observe and meticulously record changes in the patient's emotional and social status influenced by their illness and treatment response.

Maintain and submit detailed written clinical records, including initial evaluations, care plans, and daily notes as required by the agency.

Evaluate the effectiveness of medical social work interventions on both patients and their families.

Confirm on a weekly basis the scheduling of visits with patients' Case Managers to ensure seamless coordination of care.

Act as a consultant to physicians, nurses, and other health team members to help them understand social and environmental factors impacting health.

Assist patients in utilizing available resources from their families and the broader community to enhance support.

Participate actively in IDG and QAPI meetings and related activities to drive quality improvement.

Continue to offer support and guidance to the interdisciplinary team if a patient or family declines social work services.

Assess the patient's and family members' ability to cope with the dying process and provide appropriate support.

Help patients and families navigate the discharge planning process when necessary.

Take responsibility for your own professional development through ongoing education and active participation in relevant organizational activities.

Maintain current knowledge of medical social work practices through workshops, conferences, and independent research.

Requirements

Must have a Master's in Social Work (LMSW) from an accredited institution.

Must possess current hospice experience to demonstrate familiarity with end-of-life care practices.

Must be able to reliably handle and maintain confidential information in accordance with legal and ethical standards.

Must be a licensed driver with an insured automobile in good working order to travel to patient homes.

Must have the ability to communicate effectively both verbally and in writing with patients, families, and team members.

Must be able to perform duties in a variety of home environments and manage the physical demands of the job.

Must be able to lift and carry necessary equipment and supplies when required.

Must pass a background check and maintain good standing with the state licensing board.

Must be available to work on an as-needed basis to meet patient demand in the Kansas City area.

Must comply with all agency policies, procedures, and regulatory requirements related to hospice care.

Must demonstrate cultural sensitivity and respect for diverse patient populations and their families.

Must maintain professional boundaries and adhere to the ethical guidelines of the social work profession.

Must be willing to travel within the Kansas City, KS area to provide timely patient care.
