About the role
The Advanced Practice Provider owns the full clinical lifecycle for a targeted panel of homebound and palliative care patients within the Washington, DC region, delivering primary care services directly in the community. You will apply medical judgment and evidence-based practices under the oversight of a supervising physician to manage complex chronic conditions and palliative needs. This role requires you to build deep therapeutic relationships with patients and their families, operating as a trusted clinician within their home environment. You will be responsible for developing and executing comprehensive care plans that promote comfort, function, and dignity for individuals with limited mobility. The position demands strong clinical autonomy, proactive communication, and a commitment to the Ennoble Care motto that care is an honor. What you'll do
Provide high quality primary and palliative care services directly in the homes of patients across the Washington, DC area.
Conduct comprehensive patient assessments by performing detailed physical examinations and obtaining thorough medical histories to inform clinical decision-making.
Establish accurate diagnostic impressions and formulate evidence-based treatment plans tailored to the needs of homebound individuals.
Communicate effectively with internal office staff and clinical operations teams using various digital platforms to ensure seamless coordination of care.
Deliver patient-centered care that prioritizes dignity, respect, and the specific goals of each patient within a diverse panel.
Collaborate closely with patients, families, and informal caregivers to build trust and ensure that care plans align with patient values and preferences.
Order and interpret diagnostic testing as medically necessary to monitor conditions and guide ongoing treatment decisions.
Prescribe medications, medical supplies, and appropriate interventions that are medically necessary and suitable for the home setting.
Maintain strict compliance with regulatory, legal, and professional standards while practicing with clinical excellence and empathy.
Act as a key clinical resource for colleagues and operations staff, supporting continuous improvement in care delivery and patient outcomes.
Requirements
Board Certified as a Nurse Practitioner or Physician Assistant with a valid NP or PA certification in Washington, DC.
Possess strong clinical skills demonstrated through the accurate, confident assessment, diagnosis, and treatment of complex conditions.
Exhibit a positive professional demeanor and superior interpersonal skills when interacting with patients, families, and multidisciplinary team members.
Demonstrate the ability to problem solve and exercise clinical judgment independently in a home-based practice environment.
Bring 1-2 years of prior primary care experience, with a focus on geriatric populations, while hospice experience is highly preferred but not mandatory.
Maintain current licensure and certifications required for advanced practice in the state of Washington, DC, including controlled substance registrations as applicable.
Uphold the highest standards of patient confidentiality, safety, and ethical practice in all interactions and documentation.
Commit to continuous professional development and adherence to Ennoble Care policies, clinical protocols, and quality improvement initiatives.
Nice to have
Previous experience working within home-based or community-based primary care settings.
Familiarity with remote patient monitoring technologies and virtual care coordination tools.
Experience supporting palliative care and hospice patients in home environments.
Demonstrated ability to work flexibly across multiple clinical platforms and electronic health record systems.
