We are looking for a compassionate, dedicated per diem Certified Home Health Aide (CHHA) to join our growing Hospice Company for our Monmouth County, NJ region. The role is integral to delivering high-touch, in-home support that enables patients to remain comfortable and dignified in familiar surroundings. You will work closely with interdisciplinary teams, including nurses and therapists, to implement care plans tailored to each patient's needs. What you'll do

Provide care for a census of 5-6 patients during each assigned shift.

Perform duties related to personal hygiene, grooming, oral care, skin care, and nail care as part of daily support.

Assist with toileting, positioning, and transfer activities, including frequent turning and positioning of a bedbound resident or patient.

Support prescribed exercise and range-of-motion routines taught by the hospice nurse to the patient and aide.

Perform light housekeeping tasks specific to the patient's personal unit or room, including changing bed linen and laundering patient garments.

Feed the patient and assist with meals to ensure safety and nutritional needs are met.

Accurately report any observed changes in the patient's condition to the supervising nurse in a timely manner.

Monitor the home environment to maintain safety, cleanliness, and infection control standards.

Support the patient and family with emotional needs and non-medical comfort measures.

Maintain detailed documentation of services provided and any patient concerns.

Transport patients as needed within the home, using safe body mechanics and transfer techniques.

Assist with ambulation and mobility exercises to promote independence and prevent complications.

Observe and communicate any concerns regarding medication administration or medical equipment use.

Promote patient autonomy and dignity while delivering compassionate care.

Collaborate with other healthcare team members to coordinate seamless care delivery.

Requirements

In NJ, homemaker-home health aide means a person who has completed a training program approved by the New Jersey Board of Nursing and who is so certified by the Board in accordance with NJSA 45:11-23 et seq and NJAC 13:37.

Received a minimum of 75 hours of classroom and supervised practical training with at least 16 hours devoted to supervised practical training and with at least 16 hours of classroom instruction completed.

One year experience as a CNA, hospice aide or home health aide. (Hospice preferred).

At least 18 years old.

Must be a licensed driver with an insured automobile in good working order.

The ability to communicate well, both verbally and in writing.
