Housing First Coordinator

About the role

You will own the end-to-end coordination of housing stability cases within the Tenancy Preservation Program, driving person-centered plans that align clinical needs with housing goals. You will act as the primary liaison between tenants, legal teams, and community providers to ensure timely, effective interventions. This role owns the implementation of Housing First principles, translating policy into practice for individuals with complex needs. You will lead the development of actionable, measurable case plans that track progress and outcomes for court-involved households. You own the responsibility to uphold trauma-informed practices and maintain strict confidentiality in every interaction. You will use data and clinical judgment to de-escalate housing crises and advocate for reasonable accommodations. You will continuously build relationships with providers to strengthen referral pathways and resource accuracy. Your work will center on empowering clients to sustain long-term housing and community integration. You will uphold Eliot's mission of belonging, growth, and systemic change through direct practice and collaboration.

What you'll do

Coordinate intake and initial assessments for individuals referred by the Housing Court, Legal Services, the Department of Mental Health, or other authorized referral sources.

Develop individual case management and clinical plans in collaboration with the person served, identifying root causes of housing instability and setting measurable goals.

Link clients across agencies through intensive clinical assessments to identify needs and prevent homelessness.

Maintain current knowledge of local public and private resources, including entitlement programs, housing options, and community services.

Apply working knowledge of Massachusetts Housing Law, tenant and landlord rights, and the eviction process to support housing stability.

Collaborate with Court-appointed Housing Specialists to resolve tenant-landlord disputes referred to the Housing Court.

Provide consultation to the Housing Court on cases entitled to reasonable accommodations due to mental health or physical disabilities.

Assist in connecting tenants to treatment, support services, and alternative housing when stabilization of current tenancy is not possible.

Monitor and document client progress, adjusting interventions to meet evolving needs and prevent homelessness.

Support the Tenancy Preservation Program in delivering clinical consultation and resources to at-risk households.

Contribute to a team-based approach that prioritizes safety, compliance, and client empowerment.

Engage in ongoing learning about housing, behavioral health, and legal processes to maintain role effectiveness.

Communicate clearly and professionally with diverse stakeholders, including clients, providers, and court personnel.

Requirements

Bachelor's Degree in Social Work or a related discipline or two years of professional experience working with the homeless adult population.

Working knowledge and experience with evaluating eligibility criteria based on disabling conditions and reasonable accommodation options within Housing Court systems.

Experience with dispute mediation and familiarity with housing and entitlement resources is required.

Valid Massachusetts Driver's license.

Ability to work collaboratively with individuals experiencing homelessness, medical complexity, and/or untreated behavioral health concerns.

Capacity to identify barriers to obtaining and maintaining long-term sustainable housing using a Housing First approach.

Commitment to maintaining confidentiality and adhering to ethical standards in sensitive cases.

Ability to manage multiple cases and priorities in a court-mandated, deadline-driven environment.

Willingness to participate in flexible work schedules, including evenings or weekends as needed to support clients and court requirements.

Hours: Monday

Friday, 9am-5pm (Ability to work a flexible work schedule continued).

Travel: Local travel between sites may be required.
