Housing First Coordinator

About the role

You will own the end to end coordination of housing placement and stabilization for individuals experiencing chronic homelessness. You will lead the implementation of individualized Housing First plans in direct collaboration with multidisciplinary clinical and medical teams. You will champion harm reduction principles and a client centered framework in every interaction and decision. You will leverage community partnerships to open doors and sustain long term housing opportunities. You will own crisis response processes and advocate fiercely for the safety and dignity of people served. You will document all activities with precision to ensure compliance and continuous improvement. You will contribute to a culture of belonging, learning, and innovation within the Enhanced Navigation and Stabilization Team.

What you'll do

Conduct comprehensive outreach and engagement to build trust and rapport with individuals experiencing homelessness.

Develop and maintain collateral contacts with family, friends, and community stakeholders to strengthen support networks.

Perform strength based assessments to identify immediate and long term housing and support needs.

Create and update individualized service plans that align with Housing First and recovery oriented practices.

Coordinate access to permanent supportive housing, rapid rehousing, and other relevant resources.

Monitor housing retention and provide ongoing coaching to tenants and landlords to prevent eviction.

Implement crisis intervention strategies when there is a sudden change in health or behavior.

Collaborate with in the field psychiatry and medication assisted treatment providers for integrated care.

Utilize creative problem solving to remove barriers related to documentation, income, and identification.

Maintain accurate records and reports to meet regulatory, funding, and organizational requirements.

Participate in team supervision, case consultations, and quality improvement initiatives.

Promote an inclusive environment that respects diversity, equity, and cultural humility.

Serve as a resource and mentor to colleagues and interns in the homelessness services field.

Communicate proactively with supervisors regarding workload, risks, and training needs.

Requirements

Bachelor's degree in a human services field such as social work, psychology, human services, or sociology is required, or two years of case management or comparable experience working with the homeless population.

Must possess a valid Massachusetts driver's license and maintain a personally owned vehicle for essential job duties.

Ability to obtain and maintain required clearances for working with vulnerable populations as mandated by state regulations.

Commitment to the Housing First model that prioritizes moving people directly into permanent housing without preconditions.

Willingness to work Monday through Friday during regular business hours in an office and field based environment.

Reliability in maintaining scheduled appointments and meeting deadlines in a fast paced service environment.

Strong written and verbal communication skills to engage with clients, stakeholders, and team members effectively.

Professional boundaries and ethical practice aligned with organizational policies and standards.

Ability to use basic office software and electronic case management systems to perform job functions.

Willingness to participate in trainings and professional development to enhance skills and knowledge.

Tolerance for frequent exposure to individuals experiencing complex trauma, mental health challenges, and substance use disorders.

Compliance with all organizational health, safety, and confidentiality protocols.

Nice to have

A Master's degree in the healing arts is preferred.

Working knowledge of area resources and providers is a plus.

Practical notes

This is a full time position with a standard Monday to Friday, 9am to 5pm schedule.

The position may involve travel within the service area to meet clients and community partners.

Employment is contingent on successful completion of required background checks and clearance processes.
