What you'll do

• Develop and implement individualized care plans for patients experiencing homelessness, ensuring they receive appropriate medical and social services.

Requirements

• Bachelor's degree in social work, public health, nursing, or a related field; a Master's degree is preferred.
• A minimum of 3 years of experience working in community health, social services, or a related area, with a focus on underserved populations.
• Strong understanding of the challenges faced by individuals experiencing homelessness, including barriers to healthcare access.
• Excellent communication and interpersonal skills, with the ability to build rapport with diverse populations.
• Proven ability to work collaboratively within a multidisciplinary team and engage effectively with community partners.
• Familiarity with healthcare systems, social services, and resources available to homeless individuals in Los Angeles.
• Strong organizational skills and attention to detail, with the ability to manage multiple priorities and deadlines.
• Proficiency in using electronic health records (EHR) and other relevant software applications.
• Valid California driver's license and reliable transportation for outreach activities.
