About the role
As a Community Care Coordinator specializing in Street Medicine at Clinicaromero, you will play a role in addressing the healthcare needs of individuals experiencing homelessness in Los Angeles. What you'll do
- Develop and implement individualized care plans for patients experiencing homelessness, ensuring they receive appropriate medical and social services.
- Conduct outreach activities to engage with individuals living on the streets, in shelters, or in temporary housing, building trust and rapport to facilitate access to care.
- Collaborate with healthcare providers, social workers, and community organizations to coordinate services and resources for patients, including housing assistance, mental health support, and substance use treatment.
- Maintain accurate and up-to-date patient records, documenting interactions, care plans, and progress notes in compliance with organizational policies and regulations.
- Educate patients about available healthcare services, preventive care, and healthy lifestyle choices, empowering them to take charge of their health.
- Participate in regular team meetings and case conferences to discuss patient progress, share insights, and develop strategies for improving care delivery.
- Advocate for patients' needs within the healthcare system and the community, ensuring they receive the support and resources necessary for their well-being.
- Monitor and evaluate the effectiveness of care plans, making adjustments as needed to address changing patient needs and circumstances.
- Engage in continuous professional development to stay informed about best practices in street medicine and community health.
- Assist in the training and supervision of interns or volunteers who may be supporting outreach efforts and patient care.
- Participate in community events and health fairs to promote awareness of Clinicaromero's services and the importance of healthcare access for homeless populations.
- Contribute to data collection and analysis efforts to evaluate program outcomes and inform future initiatives aimed at improving care for homeless individuals.
Requirements
- Bachelor's degree in social work, public health, nursing, or a related field; a Master's degree is preferred.
- A minimum of 3 years of experience working in community health, social services, or a related area, with a focus on underserved populations.
- Strong understanding of the challenges faced by individuals experiencing homelessness, including barriers to healthcare access.
- Excellent communication and interpersonal skills, with the ability to build rapport with diverse populations.
- Proven ability to work collaboratively within a multidisciplinary team and engage effectively with community partners.
- Familiarity with healthcare systems, social services, and resources available to homeless individuals in Los Angeles.
- Strong organizational skills and attention to detail, with the ability to manage multiple priorities and deadlines.
- Proficiency in using electronic health records (EHR) and other relevant software applications.
- Valid California driver's license and reliable transportation for outreach activities.

- Bilingual proficiency in Spanish or another language commonly spoken in the community.
