About the role
You will own the strategic leadership and day-to-day coordination of the PoWER Program's Community Health Worker (CHW) activities within the Kentucky Rural Health Transformation Program, ensuring that maternal and infant health services are delivered with fidelity to the model and the Kentucky CHW scope of practice. You will act as the primary bridge between the pilot site-based CHWs and the statewide PoWER Program Team, translating high-level program design into practical, on-the-ground workflows that are consistent, measurable, and aligned with program goals. In this role, you will bring deep community-based care experience and a strong understanding of wraparound support services to guide the implementation of telehealth-enabled maternal care teams in maternity-care deserts and high-risk regions. You will promote seamless, high-quality support for mothers and infants by facilitating training, troubleshooting barriers, and ensuring that services remain patient-centered, culturally competent, and resilient. You will own the processes that turn program design into consistent practice, using your expertise to coordinate activities, document outcomes, and support continuous improvement across pilot sites. You will be a trusted liaison who builds alignment, fosters collaboration, and upholds the integrity of the PoWER model as a key member of the Program Team. You will use your skills to strengthen recruitment pipelines and help build a technology-forward, resilient health system that supports both clinical and non-clinical staff across Kentucky's rural counties.

What you'll do
Coordinate and supervise the implementation of the PoWER Program's maternal and infant health activities across pilot sites in Kentucky, ensuring adherence to the program model and the Kentucky CHW scope of practice.
Provide day-to-day leadership, technical assistance, and guidance to site-based PoWER CHWs, supporting their capacity to deliver consistent, high-quality services to mothers and infants.
Serve as the primary communication bridge between the PoWER Program Team and pilot site CHWs, facilitating alignment on goals, workflows, and expectations.
Translate program policies and evidence-based practices into clear, actionable procedures that reflect the realities of maternity-care deserts and high-risk regions.
Support the design and refinement of data collection tools and documentation processes, ensuring that performance metrics are consistently captured and used to inform program improvements.
Monitor and report on program implementation progress, identifying barriers, bottlenecks, and opportunities to strengthen service delivery and workforce deployment.
Collaborate with internal and external partners to integrate telehealth-enabled maternal care teams into the broader health system, promoting seamless coordination and continuity of care.
Lead or contribute to the development of training materials, job aids, and guidance documents that reinforce best practices and the Kentucky CHW scope of practice.
Champion the use of standardized documentation and data systems, ensuring that program activities are documented in a way that supports performance monitoring and continuous learning.
Act as a problem-solver for pilot site CHWs, helping them navigate complex cases, access resources, and maintain alignment with program standards.
Support the identification and testing of innovative approaches to strengthen rural health infrastructure, technology-enabled care, and service delivery models.
Promote the recruitment and retention of a diverse, skilled workforce by contributing to strategies that build pipelines for both clinical and non-clinical staff in rural Kentucky.
Ensure that all activities are conducted in strict accordance with the Kentucky CHW scope of practice, the PoWER Program model, and applicable regulations and policies.
Represent the PoWER Program in cross-functional meetings and workstreams, providing frontline insights that keep the program grounded in the realities of community needs.
Use feedback from CHWs, clients, and partners to refine processes, tools, and supports, driving continuous improvement across the program.
Requirements
Demonstrate experience as a Community Health Worker, public health professional, or in a similar frontline role that has provided a comprehensive understanding of community-based care and wraparound support services.
Have direct experience working in maternal and infant health, prenatal care, or postpartum support, with a clear understanding of the challenges faced in maternity-care deserts.
Possess a strong understanding of the Community Health Worker role, the Kentucky CHW scope of practice, and the value of peer support models in health care settings.
Show a proven ability to coordinate and supervise the work of others, with experience leading or supporting frontline teams in complex, community-based environments.
Have experience using data and documentation systems to track program performance, monitor outcomes, and inform continuous quality improvement.
Demonstrate strong skills in communication, facilitation, and cultural competence, with the ability to build trust across diverse communities and stakeholders.
Be comfortable working in a hybrid remote/on-site environment and using technology to support telehealth-enabled care delivery and virtual collaboration.
Be able to travel within Kentucky as needed to provide on-site support, observe program implementation, and engage with pilot sites and community partners.
Nice to have
Experience working with the CDC Foundation or other large public health organizations is preferred.
Familiarity with technology platforms used for telehealth and data management in public health settings is preferred.
Experience supporting the recruitment, training, and mentorship of community health workers is preferred.
