About the role
This role exists to strengthen community-based support for individuals facing substance use challenges. The Linkage to Care Coordinator serves as a vital connection point, guiding people from initial contact through sustained engagement with recovery resources. The position focuses on designing and managing local workflows that transform complex needs into structured, person-centered actions. Primary responsibilities include facilitating warm hand-offs, building robust community networks, and ensuring continuity of support from clinical settings to community-based recovery environments. This role reports to the CDC Foundation and works in close partnership with CDC and Nevada state authorities to advance the objectives of the Overdose Data to Action (OD2A) program. Professionals in this capacity translate public health directives into practical community strategies while maintaining a sharp focus on linkage and retention. Success in this position is defined by measurable improvements in engagement and access to care for priority populations across Southern Nevada.

What you'll do
- Coordinate care transitions for individuals with substance use disorder or co-occurring disorders, managing the full arc of engagement from initial outreach through referral completion and sustained support.
- Facilitate community connections by developing and maintaining a robust network of local resources, including healthcare providers, recovery support services, and social service agencies to address both medical and social determinants of health.
- Implement outreach strategies through proactive and reactive community engagement, including educational sessions in hospitals, urgent care centers, and community venues, alongside targeted naloxone distribution.
- Use data for action by analyzing overdose morbidity and mortality data to identify high-risk geographic areas (hotspots) and direct outreach efforts effectively in hospitals, urgent cares, and other high-need settings.
- Manage resources and logistics by overseeing supply inventories, coordinating material distribution, and ensuring outreach teams are equipped to operate consistently in the field.
- Support re-engagement by conducting outreach to individuals who have disengaged from care, meeting them in their communities to rebuild trust and re-establish supportive connections.
- Promote trauma-informed practice by educating partners and stakeholders on trauma-informed and strengths-based approaches, ensuring all interactions center dignity, safety, and empowerment.
- Collaborate with partners by coordinating schedules and activities with state and local partners to ensure alignment with OD2A strategic priorities, specifically strategies 6, 7, and 9.
- Drive continuous improvement by participating in evaluation and quality assurance activities, using feedback and outcome data to refine program methods and enhance community-level impact.
- Communicate effectively by producing clear reports and practical tools to document progress and share best practices, ensuring momentum is maintained between training and outreach cycles.
Requirements
- A minimum of three (3) years of full-time professional work experience in care coordination, case management, linkage to care, or community-based outreach within a public health or social services context.
- A Bachelor's degree in public health, social sciences, or a closely related field is preferred.
- Demonstrated knowledge of overdose prevention strategies, substance use disorder treatment models, recovery support practices, and relevant drug policy.
- Proven ability to apply trauma-informed and strengths-based frameworks in all aspects of service delivery and stakeholder interaction.
- Highly organized with exceptional time management skills, capable of delivering high-quality work independently and within a team environment under tight deadlines.
- Strong written and oral communication abilities, with a talent for translating complex information into clear, actionable guidance for diverse audiences.
- Demonstrated skill in coordinating and collaborating with a wide array of stakeholders, respecting varied perspectives and professional boundaries.
- Working knowledge of community resources and service navigation best practices is preferred.
- Prior experience working effectively in virtual settings with remote teams and community partners is required.
- Skillful use of digital communication and office software, including Microsoft Excel, Word, PowerPoint, Teams, and Zoom, for information management and virtual collaboration.
