About the Role

The Chronic Care Manager/Chronic Care Coordinator role at The NeuroMedical Center is a clinical support position dedicated to improving patient outcomes for individuals with chronic conditions. As a Chronic Care Manager, you will serve as a vital link between patients and their care teams, using the Phamily Chronic Care Management platform to coordinate daily activities and ensure that care plans are followed effectively. The position is based on-site at The NeuroMedical Center, located at 10101 Park Rowe Avenue, Baton Rouge, LA 70810, and operates Monday through Friday, 8am to 5pm. This is a full-time position with a compensation rate of $15-$17 per hour.

You will own the coordination of chronic care activities for an assigned patient population. This includes gathering and organizing patient data to identify unmet needs, prioritizing interventions, and engaging patients in their care plans. A core responsibility is improving communication between patients and their care teams while helping patients better understand and manage their health. You will handle a 500-patient caseload, with a target of 300 billable patients by the end of the month. Using the Phamily platform, you will track patient progress, document interactions, and update care plans to reflect current needs and interventions.

This role requires close attention to detail and the ability to adapt to a fast-paced healthcare environment. You will monitor patient status, flagging concerns to clinical staff as needed, and support the implementation of care plans to meet monthly goals. Documentation accuracy and timely follow-up are essential components of the position. You will also contribute to positive patient outcomes by promoting adherence to treatment plans and encouraging self-management strategies. Collaboration with nurses, physicians, and other care team members ensures a coordinated approach to chronic care.

What You'll Do

Coordinate daily chronic care tasks and patient follow-ups within the Phamily platform to ensure continuity of care.

Gather and input patient data to maintain accurate, up-to-date records that reflect current health status and interventions.

Identify unmet patient needs and prioritize appropriate interventions based on clinical guidelines and patient goals.

Engage patients in their care plans to promote adherence, self-management, and confidence in managing chronic conditions.

Improve communication between patients and their care teams to reduce barriers and enhance care coordination.

Organize care activities to support an efficient workflow that meets monthly billable targets.

Update patient documentation to reflect interventions, progress, and changes in condition or treatment plans.

Monitor patient status and escalate concerns to clinical staff when necessary to ensure timely interventions.

Support the implementation of care plans to achieve clinical and billing goals, including tracking 300 billable patients.

Utilize the Phamily platform to track patient interactions, outcomes, and compliance with care plans.

Maintain compliance with organizational policies, standards, and regulatory requirements related to patient care.

Adapt workflows to manage a high volume of patients while maintaining quality and accuracy in a fast-paced setting.

Demonstrate strong interpersonal skills when communicating with patients, families, and healthcare professionals.

Contribute to a supportive team environment focused on patient-centered care and shared clinical goals.

Requirements

Minimum of 2 years of experience in an outpatient clinic setting as a patient caregiver, educator, or advocate, ensuring familiarity with chronic care needs.

Certified Medical Assistant (CMA) preferred as validation of clinical knowledge and technical competence.

Current Basic Life Support (BLS) certification required to ensure safety in emergency situations.

Strong interpersonal, communication, and organizational skills necessary for managing multiple patient interactions.

Ability to work independently and adapt to a fast-paced environment while maintaining attention to detail.
