About the role
You will be the primary external sales representative for A Place for Mom, acting as the face of the brand within your designated hospital and skilled nursing facility territory. This role owns the end-to-end sales process, from initial outreach to final referral placement, ensuring that discharge planners and case managers view A Place for Mom as their trusted partner. You are accountable for translating complex senior care options into clear, actionable plans for families during high-stress transition moments. The position requires a disciplined approach to managing a book of accounts while consistently prospecting for new referral sources to fuel pipeline growth. You will leverage data and feedback to refine your outreach methods and optimize conversion rates across your market. Success in this role depends on your ability to build credible relationships quickly and position our services as the logical choice for discharging patients. You will work in direct partnership with internal Healthcare Senior Living Advisors to ensure a seamless handoff for families and to close the loop on referral outcomes.

What you'll do
Execute a daily outreach plan that targets new referral sources, including social workers, discharge planners, and case managers, to build a robust pipeline of qualified senior care referrals.
Generate a minimum number of qualified patient referrals per day by engaging with your assigned market plan accounts, ensuring that each interaction moves the prospect toward a potential move or care arrangement.
Maintain and update a dynamic portfolio of hospital and skilled nursing facility contacts, tracking referral status, decision-makers, and historical interactions to inform next steps.
Expand your book of accounts through strategic networking, cold outreach, and value-driven presentations that highlight the benefits of partnering with A Place for Mom.
Follow up consistently with every referral source to communicate discharge outcomes, demonstrating how your efforts contribute to better patient flow and satisfaction metrics for their facility.
Educate families and referral partners on available senior living options and home care services, clearly explaining how a Healthcare Senior Living Advisor will support them through the decision process.
Collaborate closely with your assigned Healthcare Senior Living Advisor to coordinate responses to referrals, align on messaging, and meet or exceed your monthly move-in or care placement quota.
Analyze performance reports within our CRM and internal tracking systems to identify trends, forecast future revenue, and adjust your territory strategy for maximum impact.
Pilot new sales initiatives, such as account scoring models or revised communication templates, test their effectiveness in your territory, and provide structured feedback to improve company tools and resources.
Maintain strict adherence to compliance and privacy standards when handling patient information, ensuring that all sales activities align with HIPAA guidelines and corporate policies.
Requirements
Must hold a Bachelor's degree or equivalent practical experience that demonstrates a comparable level of professional preparation.
Bring 3-5 years of progressive outside sales experience, with a proven ability to generate leads and convert them into closed opportunities.
Possess in-depth knowledge of the Senior Living industry, including how care options are discussed and decided within clinical environments.
Have documented hospital or skilled nursing facility sales experience, showing familiarity with discharge workflows and stakeholder roles.
Show a proven track record of not only meeting but exceeding sales quotas while working collaboratively with cross-functional teams.
Must be inherently relationship driven, with a strategic mindset that balances short-term wins against long-term account health.
Demonstrate a history of confidently presenting to target customers, handling complex objections, and articulating value in high-pressure situations.
Thrive in a fast-paced, constantly changing environment where independence, adaptability, and a hands-on approach are essential to daily performance.

Commit to traveling daily into your assigned accounts during the standard five-day business week, engaging stakeholders locally and face-to-face whenever possible.
Communicate effectively with internal and external stakeholders at all organizational levels, maintaining professionalism and clarity in every interaction.
Manage time efficiently to balance pipeline development, follow-up activities, and administrative tasks without sacrificing quality or accuracy.
Be technologically proficient, with advanced skills in Microsoft Office, Google Sheets, and at least one CRM platform, preferably Salesforce.
Nice to have
Preferred candidates will have prior experience selling to healthcare providers, including hospitals, rehabilitation centers, or long-term care facilities.
Experience with CRM configuration, data hygiene, and the setup of sales analytics dashboards to track key performance indicators.
Familiarity with community outreach or educational event planning for senior audiences and their families.
Practical notes
You will be in the field daily, working closely with your Regional Director to build and refine your weekly account plan.
Your field activities will include scheduled presentations, proactive calls, and spontaneous visits to meet with clinical staff and families in transition.
Base
Salary: $85-90,000
On Target Earnings: $120-145,000+ (uncapped)

This position requires daily local travel within the Sacramento area.
