This position demands rigorous adherence to compliance standards while cultivating trusted relationships with referral partners and families. Your core mission involves guiding families through complex care transitions using a consultative and empathetic approach. Success in this role is defined by your ability to generate a consistent flow of qualified referrals from clinical staff. You will design and manage a structured weekly outreach plan that prioritizes key accounts and emerging opportunities. Documenting every interaction and outcome will be essential for demonstrating value and refining our service offering. You will collaborate closely with internal Healthcare Senior Living Advisors to align referrals with appropriate care solutions. Closing moves relies on your precision in forecasting and coordination across multiple internal stakeholders. Continuous analysis of pipeline and CRM data will inform strategic adjustments to protect and grow your territory over time.
