What you'll do
Identify chronic kidney conditions early through virtual screening and patient history review. Evaluate patient status by interpreting laboratory data and diagnostic results within the telehealth platform. Manage established patients through scheduled virtual follow-up encounters and longitudinal care planning. Document encounters accurately and timely to support continuity of care in electronic medical records. Coordinate with nephrologists and other specialists to create unified care strategies for complex cases. Educate patients on lifestyle modifications, medication adherence, and disease self-management techniques. Monitor patient progress through ongoing virtual interactions and adjust treatment plans as needed. Ensure every care interaction aligns with clinical protocols, regulatory requirements, and quality standards. Utilize telemedicine technology to perform assessments and communicate securely with the care team. Support care transitions and referrals to maintain seamless patient experiences across different care settings.
Requirements
A minimum of 2 years of nephrology experience in outpatient or telehealth settings is required for this role. Candidates must hold an active Physician Assistant or Nurse Practitioner license in the state of California. Effective remote practice depends on strong written and verbal communication skills. Success in this position requires comfort with autonomous practice in a virtual care environment. Reliable high-speed internet access is necessary to support video visits and real-time data access. Familiarity with electronic health records and telemedicine platforms is essential for efficient performance. The ability to interpret clinical data and laboratory results accurately is a core job function. Professionals must adhere to strict confidentiality and compliance standards in all patient interactions.
