About the role

At Eliot Community Human Services, the Family Partner - CSA position is central to the delivery of child and family services in Lynn. Eliot's Community Service Agency (CSA) serves children and youth (through age 21) and their families who are in need of support in managing and coordinating services. The CSA is being aligned with the catchment area of established Community Behavioral Health Centers (CBHCs) to offer a seamless and effective community-based continuum of care. The Family Partner - CSA serves as a bridge between families, providers, and community partners to ensure that care is coordinated, trauma-informed, and strength-based. What you'll do

You will partner closely with a youth and their family to advance the goals of the Individual Care Plan within a community behavioral health setting. This role centers on empowering families by translating complex systems into clear, actionable steps that build resilience and long-term stability. You will operate as a trusted guide, helping caregivers navigate service coordination while reinforcing the strengths and cultural values of each household. Your work will directly support the seamless delivery of community-based services that are both trauma-informed and strength-based. You will contribute to measurable progress by fostering collaboration between families, providers, and community partners. • Work closely with the CSA team, family, and other collaterals in developing measurable family partner goals to support the family's Individual Care Plan (ICP) and related goals.
• Engage caregiver(s) in activities in the home and in the community that are designed to address one or more goals/tasks on the family's ICP.
• Assist caregiver(s) in meeting the needs of the youth through educating, supporting, coaching, modeling, advocating and guiding.
• Teach caregiver(s) how to network/link to community resources and treatment providers.
• Support caregiver(s) in advocating for services and resources to meet youth and family needs comprehensively.
• Guide, support and foster empowerment including, linkages to individual, peer/parent support and self-help groups.
• Maintain a focus on resilience, empowerment, and cultural responsiveness in every interaction with families and stakeholders.

Requirements

To ensure a strong match for this family-focused role, the following requirements are based on the stated qualifications and working conditions:

• Must be a parent, guardian or caregiver of a youth with behavioral health needs.
• Must possess a valid driver's license and reliable transportation to fulfill role responsibilities, as the role is located in Lynn and requires home and community-based engagement.
• Must hold an Associate's degree in human services or a related field.
• Must have experience in navigating child and family service systems.
• Must be bilingual in Spanish, Portuguese, or Creole to be eligible for a 6% pay increase, reflecting the language needs of the community served.

Practical notes

This is a full-time position with a standard Monday through Friday schedule from 9:00am to 5:00pm. The role is located in Lynn and requires reliable transportation due to the need for home and community-based engagement. Travel may be required across service areas to meet families and providers.
