About the role
The Hospital Relationship Manager plays a pivotal role in enhancing Abby Care's visibility and influence within hospital settings. This position serves as the main point of contact for hospital staff, focusing on educating clinical teams about our innovative family-led care platform. The role is primarily field-based, emphasizing direct interactions with hospital personnel to facilitate seamless transitions for patients moving from hospital care to home care.

What you'll do
- Establish and maintain a strong, visible presence in designated hospitals, proactively identifying and engaging with key personnel such as discharge planners, nurses, and social workers.
- Conduct informative presentations and casual discussions to educate hospital staff about the services offered by Abby Care, ensuring they are well-informed about patient eligibility for our care platform.
- Drive the generation of qualified patient referrals by meeting established volume targets, ensuring a consistent flow of potential patients for our services.
- Work collaboratively with hospital teams and Abby Care's intake department to facilitate smooth and efficient patient admissions from the hospital setting to home care services.
- Keep meticulous records of all interactions with hospital facilities, tracking the development of relationships and managing your referral pipeline effectively.
- Analyze growth metrics and referral trends using our Customer Relationship Management (CRM) system to inform strategic decisions and improve outreach efforts.
- Participate in regular meetings with hospital staff to discuss patient care strategies and address any challenges related to patient transitions.
- Serve as a trusted resource for clinical teams, providing assistance with patient placement and addressing any concerns that arise during the transition process.
Requirements
- A minimum of four years of experience in a hospital or healthcare-related environment is essential.
- Proven track record of successfully building and nurturing relationships with various hospital teams, including discharge planners, social workers, and nursing staff.
- Hands-on experience working directly on hospital floors, nursing stations, and within discharge planning areas is required.
- Ability to establish trust and credibility with clinical teams, becoming a go-to resource for addressing patient placement and transition issues.
- Strong understanding of clinical settings and the processes involved in transitioning patients from hospital care to home care.
- Excellent communication and interpersonal skills, with the ability to engage effectively with diverse healthcare professionals.
Nice to have
- Familiarity with home care services or family-led care models is a plus.
- Previous experience in a sales or business development role within the healthcare sector would be advantageous.
- Knowledge of healthcare regulations and compliance standards related to patient care transitions.
- Proficiency in using CRM systems and other digital tools for tracking and managing relationships.

- Strong organizational skills and the ability to manage multiple relationships and tasks simultaneously.
- Proficient in Microsoft Office Suite (Word, Excel, PowerPoint) and other relevant software applications.

- Excellent problem-solving abilities and a proactive approach to addressing challenges in patient transitions.
