In this pivotal role, you will engage directly with hospitals to ensure seamless transitions for patients moving to home care services. As the main liaison for clinical staff, your responsibilities will include educating them about Abby Care's offerings and fostering robust relationships. The focus will be on generating patient referrals and facilitating smooth transitions from hospital settings to home environments.

What you'll do
- Establish and maintain a regular presence in designated hospitals, identifying and connecting with key personnel such as discharge planners, social workers, and nursing staff.
- Conduct educational sessions and informal discussions with hospital staff to inform them about Abby Care's services and identify patients who would benefit from home care.
- Proactively seek out eligible patients, developing a consistent pipeline of referrals, with your performance metrics linked to the volume of referrals generated.
- Work collaboratively with hospital teams and Abby Care's intake department to facilitate efficient patient admissions from hospital settings to home care.
- Keep thorough records of all interactions with hospital facilities, tracking relationship development and referral pipeline status using a customer relationship management (CRM) system.
- Participate in regular meetings with the business development team to discuss strategies for improving referral rates and enhancing relationships with hospital staff.
- Address any concerns or questions from hospital staff regarding Abby Care's services, providing timely and accurate information to foster trust and reliability.
- Monitor industry trends and changes in healthcare regulations that may impact patient transitions, adjusting strategies accordingly to maintain compliance and effectiveness.
Requirements
- A minimum of 4 years of experience in a hospital or healthcare environment, demonstrating a solid understanding of the industry.
- Proven track record of building and nurturing relationships with various hospital teams, including discharge planners, social workers, and nursing staff.
- Experience working directly within hospital settings, such as patient care floors, nursing stations, and discharge planning areas.
- Ability to serve as a vital resource for clinical teams, assisting with patient placements and ensuring smooth transitions to home care.
- Strong knowledge of hospital workflows, discharge planning processes, and the intricacies of patient transitions to home care services.
- Excellent communication and interpersonal skills, with the ability to engage effectively with diverse healthcare professionals.
