This position represents a hybrid opportunity where a Registered Nurse functions simultaneously as a direct care coordinator and a clinical educator within a mission-driven, AI-native platform. The hire will own the end-to-end management of a diverse patient caseload, ensuring high-quality, efficient, and compassionate care delivery in home settings. Concurrently, they will design and execute hands-on training that empowers the next generation of Home Health Aides, directly shaping the clinical workforce of the future. What you'll do
- Manage a diverse patient caseload to ensure quality care and efficient coordination across clinical teams.
- Develop and implement individualized care plans based on comprehensive assessments and measurable patient goals.
- Coordinate care activities with Primary Care Physicians and internal stakeholders to align treatment strategies.
- Coach and oversee Home Health Aides to ensure strict adherence to clinical standards and best practices.
- Educate patients and their families on complex health needs and detailed treatment plans in clear, actionable terms.
- Maintain accurate, timely, and compliant electronic charting with a preference for real-time documentation.
- Ensure all documentation meets rigorous regulatory and Abby Care standards, with strict adherence to a 24-hour turnaround time.
- Complete all administrative tasks thoroughly, including but not limited to mileage reimbursements and related logistics.
- Design and implement engaging skill training classes specifically tailored for Home Health Aide students.
- Support students directly during in-person training sessions conducted at long-term care facilities.
Requirements
- Possess an Associate's or Baccalaureate degree in Nursing from an accredited institution.
- Hold a current, valid Registered Nurse license in Pennsylvania or a Compact/Multi-state unencumbered license.
- Demonstrate a minimum of 2 years of clinical experience as a Registered Nurse in any setting.
- Show at least 1 year of dedicated home health experience prior to start date.
- Maintain a valid driver's license, provide reliable personal transportation, and carry current auto insurance.
- Exhibit the ability to work flexible hours, including nights, weekends, and variable scheduling as operational needs require.
- Demonstrate strong communication and interpersonal skills essential for collaborating with patients, families, and multidisciplinary teams.
