Requirements

Holding a current and unencumbered Registered Nurse licensure is mandatory for this position.

Maintaining active status within the Nurse Licensure Compact (NLC) if applicable to your practice region is required.

Demonstrating a minimum of five years of cumulative nursing experience with a distinct focus on care management principles and chronic condition management.

Showing proven skill in motivational interviewing techniques, comprehensive patient education, and robust patient advocacy.

Proving the ability to function effectively and independently within a remote work structure while sustaining strong communication and collaboration with the team.

Demonstrating comfort and proficiency with technology, including the ability to navigate multiple Electronic Medical Record (EMR) systems efficiently.

Residing within St. Tammany Parish, Louisiana is required to satisfy local compliance regulations and coverage obligations.

Possessing a valid driver's license and reliable transportation is necessary to support occasional travel requirements.

Nice to have

Earning a Bachelor's of Science in Nursing (BSN) or a closely related field is a preferred qualification.

Holding a Specialty Certification in Care Management, such as CCM, is highly valued.

Demonstrating fluency in multiple languages to support and serve diverse patient communities effectively.

Practical notes

This is a remote position, but candidates must reside in St. Tammany Parish, LA.
