About the role

Vynca is looking for a dedicated Social Worker to join our Clinical Care Delivery Team. This role involves providing essential psychosocial support, counseling, and care coordination for seriously ill patients. You will work within an interdisciplinary team, using a hybrid model of in-person visits and telehealth to deliver comprehensive care.

What you'll do

• Perform comprehensive psychosocial assessments to identify patient and family needs and strengths.
• Offer grief and bereavement support, including anticipatory grief counseling and funeral assistance.
• Screen for and address mental health concerns such as depression, anxiety, and caregiver stress.
• Evaluate safety risks, including suicide ideation, abuse, neglect, and unsafe living conditions.
• Participate in weekly interdisciplinary team meetings to discuss patient care plans.
• Manage medical social work cases for a specific group of patients and their families.
• Address psychosocial, practical, financial, and legal concerns for patients and their support systems.
• Help patients and families access and utilize community resources.
• Guide patients and families through social and healthcare systems.
• Mediate conflicts between families and care networks.
• Complete financial needs assessments and assist with navigating Medicare, Medicaid, Disability, Social Security, and Veterans Affairs benefits.
• Assess spiritual and medical needs, communicating them promptly to the interdisciplinary team.
• Build and maintain relationships with community resources.
• Develop social work plans of care, including identifying problems, goals, interventions, and visit frequency.
• Facilitate advance care planning, including completing advance directives and POLST forms.
• Complete all documentation within 48 hours.
• Coordinate with the interdisciplinary team and external agencies, including facilitating hospice referrals.
• Assist with community outreach and education about Vynca services.
• Undertake other duties as directed by the Clinical Manager or Chief Medical Officer.
• Travel throughout the assigned New Jersey territory for in-person patient visits, training, or company events.

Requirements

• Active, unrestricted New Jersey Social Work license (LCSW), with willingness to obtain licensure in other states.
• 2+ years of health care social work experience.
• MSW in Social Work or BSW with equivalent experience.
• Experience in hospice, palliative care, oncology, or home health.
• Understanding of cultural competency with the target population.
• Strong oral and written communication skills, with positive interpersonal skills.
• General computer skills and working knowledge of MS Office, G-Suite, and the internet.
• Proficiency in Electronic Medical Record (EMR) systems and other business systems like Salesforce.
• Available to work Monday through Friday 8:30am to 5:00pm Eastern Time, with flexibility for potential evenings and weekends.

Nice to have

• Palliative care certification.
• Bilingual (English/Spanish).
• CHPN, C-ASWCM, or similar certification, or ability to obtain one.

Practical notes

This is a hybrid role requiring travel throughout northern New Jersey for in-person patient visits, combined with telehealth care across NJ and other states. Candidates must reside within 25 miles of the assigned territory. The hiring process may include a phone screen, online assessment(s), interview(s), an offer, and background/reference checks. A background check, potentially including a drug test or other health screenings, is required before employment. Employees in patient-facing roles must be vaccinated against influenza, with religious or medical accommodations considered.
